PNAS:肠道群菌或决定乙型肝炎急慢发作

人类和驻留在我们的身体里的细菌处于共生关系。这些细菌和它们各自的环境被称为微生物。生活在肠道里的微生物一直特别令人感兴趣,因为最近的研究表明肠道菌群的组成与能影响免疫系统的某些疾病之间有相关性。

研究人员发现成年小鼠都表现出清除病毒的能力,而年轻小鼠却没有。12周大的成年小鼠感染HBV后,5周后病毒清除。而年轻的小鼠感染26周后,仍然有病毒。

研究人员想检测肠道菌群的组成是否与HBV感染有相关性。他们对一部分成年小鼠使用抗生素治疗。其他成年和年轻小鼠都未使用抗生素治疗。一般情况下小鼠在6到8周之间肠道菌群出现实质性的变化,9周后趋于稳定。抗生素治疗的小鼠,治疗过程中几乎检测不到肠道菌群直到治疗结束后4天。值得注意的是,治疗结束4周后,肠道微生物依然没有回到类似于未治疗的成年小鼠的水平。

未接受抗生素治疗的成年小鼠感染HBV后,HBV在6周后检测不到。使用抗生素的成年小鼠,60%显示6周后HBV存在。在年轻小鼠中,90%的显示HBV在6周后仍在。6个月后,53%的抗生素成年鼠和80%的年轻小鼠仍然感染了HBV。这些结果表明,肠道微生物起着对HBV重要免疫反应。

分子研究表示,肠道菌群组成物可能影响到信号通路激活TLR4基因。TLR4沉默的小鼠,不分年龄,能够8周内清除HBV并且有明显抗体存在。

这项研究的结果表明,肠道菌群的组合物起着对HBV肝脏免疫反应的作用。年轻的老鼠可能会更容易发展为慢性乙肝,因为他们的肠道菌群尚未稳定。成年人,其肠道微生物已经稳定,更有可能发展为急性乙肝; 但是成年小鼠肠内微生物被抗生素打乱,对HBV感染反应可能同年轻小鼠一样。
