单倍型造血干细胞移植为患者带来希望

北京2016年10月21日电 /美通社/ -- 2016年10月6~7日,由中华医学会血液学分会、北京大学血液病研究所联合主办,赛诺菲等公司支持的“‘移植的艺术-- 聚焦单倍型移植学术会议暨2016年中华医学会血液学分会单倍体移植学术会议在北京大学人民医院隆重召开,百余位海内外学者参会。会上,来自中国、美国、法国、德国、意大利、日本、韩国等多位造血干细胞移植领域的知名专家和与会者共同分享了单倍体造血干细胞移植近年来的发展与成就。

造血干细胞移植以往要求HLA配型全相合,只能在同胞之间或骨髓库找到,配型成功几率只有25%[1],很多患者因没有同胞供者或等待骨髓时间过长而死亡。而单倍体移植技术,以国际原创的单倍体移植治疗体系-“北京模式”为例,只要求HLA配型部分相合,这样父母、子女、表兄弟、叔叔等亲属都可以成为合适供者,该技术将为需要进行造血干细胞移植但又找不到全相合配型的患者,在短时间内找到供者,提供了更多救治机会。

在此次“‘移植的艺术-- 聚焦单倍型移植会议上,围绕单倍体移植,我国及多位外知名专家就移植适应症进展、新的移植模式探讨、移植合并症及复发的防治新策略、基础研究进展及移植相关临床试验等话题分别做了精彩的主题报告。大会主席、北京大学血液病研究所所长、北京大学人民医院血液科主任、中华医学会血液学分会主任委员黄晓军教授在开幕致辞与大会报告中介绍了中国造血干细胞移植尤其是单倍型移植事业的创新与发展。

黄晓军教授指出,造血干细胞移植新的治疗体系 -- “北京模式”,开启了单倍体移植的先河,使“父母供子女”、“子女供父母”等移植方式成为可能。“北京模式”成为国际公认的模式,得到了全国50余家移植中心及意大利、以色列等十余家海外中心应用,全球50%以上单倍体移植病例均使用这一技术。

2015年6月,由黄晓军教授主导、北京大学人民医院王昱副主任医师和南方医科大学南方医院刘启发教授为第一作者的论文“急性髓系白血病第一次缓解期单倍体与同胞全合造血干细胞移植疗效对比”发表在顶级学术期刊Blood杂志。该项研究发现,对于第一次缓解期的中高危急性髓系白血病,亲缘单倍体移植与同胞相合移植等同疗效。

北京大学人民医院牵头的多中心前瞻临床试验显示:进入临床试验研究231例单倍体及219例同胞相合病例,3年无病生存率、3年总生存率、3年累积复发率、非复发死亡率均相差无几,单倍体移植获得与同胞相合移植等同的疗效,进一步确立了其一线治疗地位,解决供者来源匮乏问题。

参会专家之一,美国MD Anderson移植与细胞治疗中心主任、国际骨髓移植研究中心(CIBMTR)前任主席、美国骨髓移植学会(ASBMT)终身成就奖获得者Richard Champlin教授表示,中国单倍体移植技术的发展与成就是有目共睹的,已经成为大家公认的单倍体造血干细胞移植体系之一。此次会议的举办对了增加国际同行间的交流,探讨如何更好地应用单倍体造血干细胞移植造福患者,非常有好处。

除了白血病,单倍体移植技术在再生障碍性贫血、淋巴瘤、多发性骨髓瘤、骨髓增生异常综合征、地中海贫血等恶性血液病方面应用也很广泛。以往,异基因造血干细胞移植仅限于白细胞分化抗原相合同胞(即兄弟姐妹)或骨髓库非血缘供者。但同胞供者HLA配型相合机率为25%,骨髓库找到无关供者的机率仅为1/5~10万。我国既往特有的独生子女政策,使找到同胞供者的机率大幅降低。

供者来源匮乏同样是世界性难题。来自美国的知名造血干细胞移植专家Robert Gale教授在会议间歇向记者表示,单倍体造血干细胞移植是目前全球都在推动发展的移植技术,它可以作为替代性供者解决很多临床实际问题。中国在这方面做得很出色,有一些很好的研究出来,显示单倍体的疗效可以与全相和移植类似。但单倍体移植仍有一些问题亟待探索,国际性的多中心临床研究可能是一个解决方案。

此次“‘移植的艺术-- 聚焦单倍型移植学术会议暨2016年中华医学会血液学分会单倍体移植学术会议的圆满召开,欣喜地看到为了让患者有更多救命的机会,我国的血液病工作者研究出了HLA配型不全相合亲属单倍体造血干细胞移植“北京方案”获得了国际同行的广泛认可与赞许。更令人鼓舞的是,单倍体移植技术使得几乎所有患者均有至少一位HLA部分相合的亲属供者,包括父母、子女、同胞、表亲,几乎有了100%的救命机会。
