肿瘤免疫治疗可以说是肿瘤治疗近20年来最大的突破。在众多实体瘤中均获得不俗的效果与进展。以PD-1/PD-L1等为代表的免疫检查点抑制剂更是掀起了免疫治疗的风潮,改变了肿瘤治疗的格局。 然而,肿瘤治疗的有效性是患者与临床医生最关心的问题,PD-1/PD-L1等免疫治疗同样也面临诸多挑战,如对免疫治疗具有反应的人群比例仍然不高,虽然通过结合化疗,或放疗能够提升免疫治疗的应答率,但是离人们攻克肿瘤的预期仍然存在距离。另外,还有大量的“冷肿瘤”,使得免疫治疗仍然束手无策。同时,即使是免疫治疗,仍然存在大量患者出现耐药的现象。 因此,基础和临床都不约而同地跳出现有的免疫治疗框架,聚焦到 “全新理论+全新设计+全新数据+全新靶点”的“四新”免疫治疗药物上。目前这四新的将主导未来肿瘤免疫治疗20年的发展。 一、从内部阻断PD-1信号,双特异抗体招募磷酸酶优于传统PD-1抗体效果 一直以来大家对PD-1信号通路的理解是:PD-1在配体PD-L1或PD-L2的结合下胞内段特定酪氨酸发生磷酸化,进而招募下游分子抑制免疫细胞的功能。而基于此设计的PD-1/PD-L1阻断抗体都是致力于阻断二者之间的结合,从
