噩梦可引起青少年自杀和非自杀性自伤

自杀尝试和非自杀性自伤严重影响了当代青少年的健康成长,其中非自杀性自伤(non-suicidal self-injury,NSSI)行为是指个体不以死亡为目的,采取一系列反复、故意、直接伤害自己身体,且不被社会所允许的行为,常常伴有一种迫切感与渴求感。
有研究认为睡眠与精神疾病有着密切联系,因此,中国山东大学的 Liu 等研究者采集了睡眠相关的不同变量:睡眠时长、睡眠质量、失眠症状和噩梦,与青少年自杀行为和 NSSI 的关联及其独立相关性进行了分析和研究。
这是一项在山东开展的前瞻性纵向研究,参与者为 7072 名青少年,首次评估进行于 2015 年 12 月,再评估进行于一年后。该研究使用自我管理问卷评估了自杀行为、NSSI、夜晚睡眠时间、失眠症状、睡眠质量、噩梦、易冲动、抑郁,以及家族人口学相关信息,并采用逻辑回归分析检验了睡眠相关变量与自杀行为和 NSSI 的关联。研究于 2019 年 5 月发表于 J Clin Psychiatry。
经过一年的随访,其中有 190 名(2.7%)参与者有过自杀尝试,621 名(8.8%)参与者有过 NSSI。
统计发现,失眠症状和噩梦频率(每月次数)的基线值与一年后的自杀尝试和 NSSI 明显相关。调整协变量(青少年和家族人口学、抑郁、易冲动、首次自杀尝试/NSSI、一年中噩梦发生频率)之后显示,自杀尝试的 OR 值 = 1.96(95% CI,1.15-3.33),NSSI 的 OR 值 = 1.52(95% CI,1.10-2.08)。
研究者进一步调整失眠症状、睡眠质量和睡眠时长之后,噩梦频率和后续自杀尝试以及 NSSI 的关联没有明显改变。失眠症、睡眠时间短,和睡眠质量差与后续的自杀尝试与 NSSI 无明显独立相关性。
