KIDNEY INT:肥胖可以预测一般人群慢性肾脏病的发病情况!

较高的BMI可以预测无肾衰竭(CKD1-2期)以及CKD 3期或更高患者的蛋白尿发病,只有在肥胖的患者中其效果才显著。由此可见,该研究结果可能提示改善风险分层,并建议在一般人群中进行体重控制。

肥胖与慢性肾脏疾病(CKD)是公共卫生的重点,并且共享核心的病理生理机制。然而,较高的体重指数(BMI)是否会增加新发CKD的风险目前仍不清楚。为了评估在一般成年人群中BMI在CKD发病的预测中所起的作用,研究人员进行了一项系统回顾和荟萃分析,并将分析结果发表于肾脏病领域权威杂志kidney international上。

研究人员检索了PubMed和ISI Web of Science等数据库,查找了在2000年1月至2016年8月期间发表的文章,并且没有语言的限制。研究人员选择了在基础肾功能正常的普通人群成年个体中进行并报道了低肾小球滤过率(eGFR)(低于60 ml/min/1.73m2)和/或蛋白尿(测验结果为1 +或蛋白肌酐比值超过3.4mg/mmol)风险相比于肥胖、超重或BMI作为连续值的风险比、比值比或相对风险的研究。

该荟萃分析共纳入了39个队列630677名参与者,平均随访6.8年。肥胖导致发生低eGFR(1.28,1.07-1.54[I2:95%])和蛋白尿(1.51,1.36-1.67 [I2:62.7%])的相对风险、95%置信区间和异质性增加。BMI增加1单位也与更高的低eGFR(1.02,1.01-1.03[I2:24.3%])和蛋白尿(1.02,1.00-1.04[I2:0% ])风险相关。相反,超重不能预测低eGFR(1.06,0.94-1.21[I2:50%])或蛋白尿(1.24,0.98-1.58[I2:49.4%])的发病。

因此,较高的BMI可以预测无肾衰竭(CKD1-2期)以及CKD 3期或更高患者的蛋白尿发病,只有在肥胖的患者中其效果才显著。由此可见,该研究结果可能提示改善风险分层,并建议在一般人群中进行体重控制。
