糖尿病患者心血管危险性评估依赖传统危险因素。然而,许多新的生物标志物被认为是心血管疾病的独立预测因子,这可能显著改善糖尿病患者的风险预测。近日,Scientific reports 杂志上发表了一篇研究文章,研究人员旨在通过调查135种进化的生物标志物来改善糖尿病患者心血管风险的预测。基于选定的生物标志物,研究人员设计了一种适用于长期心血管死亡率的预测算法。研究人员前瞻性地纳入了路德维希港心血管风险和健康研究(LURIC)的864例糖尿病患者。该研究平均随访9.6年,研究人员采用基于Cox回归分析的自举方法选择独立危险因素。以下七个变量被选定进入最终的多变量模型:NT-proBNP、年龄、性别、肾素、糖尿病病程、Lp-PLA2和25-OH维生素D3。基于上述变量的风险评分显示10年心血管生存的鉴别效能优越,C-统计量为0.76(P<0.001),明显优于了UKPDS风险评估(C-统计量为0.64,P<0.001)。相比于UKPDS风险评估,净重分类证实个人风险预测显著改善了22%(95%可信区间:14-30%)(P<0.001)。由此可见,基于传统心血管危险因素和新的生物标
