由加利福尼亚州森尼维尔市希菲尔德公司研制的一种名为GeneXpert的方法——并于2010年得到了世界卫生组织(WHO)的大力支持,能够在两三个小时内准确测定结核分枝杆菌的脱氧核糖核酸(DNA),但这种方法需要专门的设备以及人员培训,因此对于农村地区或发展中国家而言有些不切实际。

为了加速这一过程,斯坦福大学化学家Jianghong Rao与得克萨斯农工大学健康科学中心微生物学家Jeffrey Cirillo研制了一种名为CDG-3的化学制品,这种化合物在被结核分枝杆菌的一种名为BlaC的酶分解后便会发光。研究人员发现,利用这种方法,他们在1毫升样本中便可以检测到10个细菌。

研究人员随后利用采自得克萨斯州的50个痰液样本对这种方法进行了测试。结果表明,该方法可以正确识别所有在显微镜下可见的结核分枝杆菌样本,同时还能够识别80%不可见的传染源。在对非TB患者进行测试时,CDG-3探针的诊断准确率达到了73%。研究人员在7月3日的德国《应用化学》网络版上报告了这一研究成果。

新泽西州纽瓦克市罗格斯大学传染病专家David Alland表示,尽管这项测试有着令人印象深刻的敏感性,但健康人有27%的几率被误诊为TB的事实意味着它将成为一种最有用的分诊方法。例如,在一些偏远的山村,这种方法可以用来排除那些不需要治疗的人们,以便为剩下的人用更昂贵和精确的方法——例如GeneXpert——进行更进一步的临床测试创造条件。在2013年发布的一项研究结果中,Alland及其同事通过计算得出,一套假设的分诊系统能够削减30%到40%的开销。

Rao和Cirillo如今正在与得克萨斯州的诊断公司GBDbio合作,开发一种靠电池供电的便携装置。该装置将能够识别CDG-3被分解时发出的荧光。该公司首席执行官Michael Norman表示,他们希望能够在2015年完成研制工作并顺利上市。他预测,一次单项测试将花费5美元,并且用不了30分钟便能够给出诊断结果。

阿尔伯克基市新墨西哥大学毒理学家Graham Timmins说:“这里为更快速的分诊测试留下了许多空间。”他指出,从健康的角度而言,假阴性(导致感染者被送回家)比假阳性(这会使健康人接受不必要的治疗)更令人担忧——尽管过度使用抗生素会导致耐药性。他称赞这篇论文实现了从实验室到临床的“巨大飞跃”,但他同时表示,该项技术还需要对更加广泛的人群进行测试。
