尽管之前有观察性研究显示,PPIs的应用与社区获得性肺炎相关,但这些研究存在一定的限制性。比如观察对象包括胃食管反流(GERD)患者,而GERD本身就是肺炎的独立危险因素。为减少上述偏倚造成的影响,来自加拿大的Filion博士等做了一项研究,其结果发表于2014年的Gut杂志上。研究发现,应用PPIs或组胺受体拮抗剂(H2RAs)并不增加社区获得性肺炎的发生风险。
研究回顾性分析了包括加拿大6个州、美国Market Scan及英国全学科研究数据库在内 8个部分的数据,观察对象为使用PPIs或H2RAs来预防NSAIDs所致消化性溃疡的患者,这类患者合并GERD的可能性较小。纳入其中在1997年1月-2010年3月被开具NASIDs且年龄大于40岁的患者,并根据其是否使用PPI或H2RAs进行分组,比较他们6个月内发生需住院治疗的社区获得性肺炎的风险。
结果显示,使用和为未使用PPIs的患者人数分别为96870名和4141634名;使用和未使用H2RAs的患者分别为47344名和4342733名。使用PPI的患者更多为女性、年龄较长、合并其他疾病、接受过流感或肺炎疫苗。使用与未使用PPIs或H2RAs的患者6个月内发生需住院治疗的社区获得性肺炎的风险见下表。
