  背景 有人提出,在携带功能缺失CYP2C19等位基因(与氯吡格雷转化为其活性代谢产物减少相关)的人群中,氯吡格雷在降低心血管事件发生率方面有可能效果欠佳。

  方法 我们对来自两项大规模随机临床试验的病人进行基因型分析,这两项试验显示,在急性冠脉综合征病人和房颤病人中,与安慰剂相比,氯吡格雷降低心血管事件发生率(主要疗效转归)。病人接受了3种单核苷酸多态性(*2、*3、*17)的基因型分析,以确定主要的CYP2C19等位基因。

  结果 在5059例接受基因型分析的急性冠脉综合征病人中,无论遗传上已确定的代谢表型如何,与安慰剂相比,氯吡格雷显著降低主要疗效转归的发生率(异质性检验 P=0.12)。在携带功能缺失等位基因杂合子或纯合子的病人与未携带等位基因的病人中,氯吡格雷降低主要疗效转归发生率的效果相似[在携带者中发生率为:氯吡格雷组8.0%对安慰剂组11.6%,氯吡格雷组的风险比为0.69,95%可信区间(CI)为0.49~0.98;在未携带者中发生率为 9.5%对13.0%,风险比为0.72,95%CI为0.59~0.87]。相反,功能获得等位基因携带者从氯吡格雷治疗中获得的与安慰剂相比的益处大于未携带者(在携带者中主要转归发生率为7.7%对13.0%,风险比为0.55,95%CI为0.42~0.73;在未携带者中发生率为10.0%对 12.2%,风险比为0.85,95%CI为0.68~1.05,相互作用检验P=0.02)。按照基因型亚组分析,氯吡格雷对出血的影响没有差异。在 1156例接受基因型分析的房颤病人中,有关疗效或出血,在研究治疗与代谢表型、功能缺失等位基因携带状态或功能获得等位基因携带状态之间,并无相互作用的证据。

  结论 在急性冠脉综合征或房颤病人中,无论功能缺失CYP2C19携带状态如何,与安慰剂相比,氯吡格雷的疗效是一致的。
