澳大利亚科学家Richard D. Bagnall教授为首的研究者前瞻性地收集了2010-2012年澳大利亚和新西兰1~35岁所有心脏性猝死人群的临床、人口统计学及尸检信息。对全面尸检之后无法确定病因的病例,分析了至少59种心脏基因以期发现疾病相关的致病基因。这项研究被发表在新英格兰医学杂志(NEJM)上的。

该研究共计确定了490例心脏性猝死病例。1~35岁的病例中,心脏性猝死年发生率为1.3/10万,其中72%的病例为男性。31~35岁人群中,心脏性猝死的年发生率最高(3.2/10万);16~20岁人群中,病因不明的心脏性猝死的年发生率最高(0.8/10万)(图1)。

心脏性猝死的最常见病因是冠状动脉疾病(占24%)、遗传性心肌病(占16%)。31~35岁的病例中,冠状动脉疾病最常见,而在其他年龄的人群中,病因不明的心脏性猝死占比最高(占40%)(图2)。

病因不明的心脏性猝死病例年龄较轻,多在夜间死亡。对病因不明的心脏性猝死的113例病例进行基因检测,确定31例(27%)存在有临床意义的心脏基因突变。随访期间,在出现病因不明的心脏性猝死的病例家族中,13%的家族成员被临床诊断为遗传性心血管病。

因此,这项与成千上万的年轻的澳大利亚人和新西兰人心源性猝死相关的基因,这将被用于预防高危家庭成员更多的死亡。

BMJ Open:白领比蓝领心脏性猝死风险更高

工作环境会影响心血管疾病风险,研究者进行了一项研究,探究职业是否会影响心脏性猝死(SCD)风险。该研究将SCD病例(18-65岁)与其他原因死亡的对照组进行比较。通过死亡证信息和标准职业分类获取参与者职业(白领、蓝领、家庭主妇)。该研究共纳入了646例SCD,其中74%为男性,对照组有622例,其中73.6%为男性。研究数据显示,SCD组中白领比例更高(52.7% vs 43.7%; p=0.01

美敦力启动IMPROVE SCA试验:关注心脏性猝死(SCD)1.5级预防

潜在的高风险患者,应尽早植入埋藏式心律转复除颤器(ICD)。   近日,世界领先的医疗科技公司美敦力宣布启动一项名为IMPROVE SCA的全球临床试验。这项临床试验将在全球10个国家和地区的100多家医院开展,共同研究1.5级预防对心脏性猝死(SCD)防治的临床意义。   据了解,该项研究共有包括阜外医院、四川大学华西医院等25家国内医院参与。
