每年全世界都有超过一百万婴儿出生自感染HIV的孕妇,这些婴儿中大部分都会“逃脱”HIV的感染,但却会遭受母源性HIV感染带来的伤害或影响,相比未遭受HIV暴露的婴儿而言,遭受HIV暴露但未感染的婴儿的死亡率往往是前者大约两倍;母乳喂养会给感染HIV或未感染HIV的婴儿带来一定健康效益,因此母乳喂养结合母亲的抗逆转录病毒治疗是目前在资源匮乏区域被推荐给HIV阳性母亲哺育后代的特殊方法。

研究者假设,感染HIV母亲机体中微生物组和母乳中低聚糖组成的改变会影响婴儿的健康,低聚糖是母乳中第三大组分,其并不会被机体所消化,但却会为婴儿机体的微生物组提供营养,同时还会帮助婴儿机体免疫系统的发育。婴儿机体中健康的微生物组可以大大影响其机体代谢和免疫力的发育,而未感染HIV但却暴露HIV的婴儿机体中微生物组的改变或许会增加其患病及死亡率。

为了检测上述理论,研究者从海地太子港招募了50对母亲和婴儿来进行研究,研究者将研究对象分为HIV阳性和阴性组,同时对来自母亲和婴儿机体的微生物组样本进行分析;文章第一作者Jeffrey M. Bender说道,相比较在所有母亲机体中鉴别出的大部分一致的微生物群体而言,暴露于HIV但未感染HIV的婴儿(母亲HIV阳性)机体中的微生物组同HIV阴性母亲所生的婴儿机体的微生物组截然不同,同时研究者还指出,基于母亲HIV的感染状态,婴儿粪便细菌的组成或许也会发生明显改变。

研究者发现,感染和未感染HIV的母亲机体的细菌群落较为相似,因此婴儿机体肠道中正常细菌生态群落的失衡或不健康改变或许并不能被母亲-婴儿微生物组的转移来完全解释,相反,感染HIV的母亲母乳中低聚糖成分的改变或许对于婴儿机体微生物组的建立有着明显的负面影响。

研究者Bender表示,未成熟且失调的微生物组会潜在中和影响婴儿机体免疫系统的发育,而感染HIV母亲机体微生物组组成的轻微干扰及母乳中低聚糖组成的差异或许就可以解释婴儿机体微生物组所产生的改变。因此为婴儿提供重要的有益菌或特殊的牛奶寡糖类物质或许就可以潜在改善婴儿的机体发育和健康状况。
