Sci Trans Med:CRISPR编辑人类脂肪细胞,可防止饮食诱导的肥胖

已有的研究显示,激活BAT可增加能量消耗,其活性与体重指数和脂肪质量成反比,使BAT成为抗肥胖疗法的诱人目标。

目前,肥胖症和代谢综合症正在全世界范围内迅速增加,其会导致高发病率和死亡率。制定肥胖症及其并发症的预防和治疗策略对卫生保健界来说是非常重要的。在哺乳动物中,棕色脂肪组织(BAT)和白色脂肪组织(WAT)都有助于系统性的能量平衡;然而,它们的解剖学、形态学和功能是完全不同的。WAT是储存多余燃料的主要场所,含有单个脂滴,而BAT是特异性的能量消耗场所,具有多个脂滴。

已有的研究显示,激活BAT可增加能量消耗,其活性与体重指数和脂肪质量成反比,使BAT成为抗肥胖疗法的诱人目标。然而,至今为止,基于细胞的疗法在人类中的治疗效果仍不清楚。

最近,研究人员在Science Translational Medicine杂志发文,其通过使用CRISPR-Cas9-SAM-gRNA技术,在人类白色脂肪前体细胞中激活内源性解偶联蛋白1的表达,来创建人类棕色脂肪样细胞(HUMBLE)。

为了研究HUMBLE细胞是否可以防止小鼠在饮食诱导的肥胖中出现代谢紊乱,研究人员监测了移植前2周用45%高脂饮食(HFD)喂养的受体小鼠的代谢表型,并在移植后继续相同的饮食4周。接受HUMBLE和棕色对照脂肪细胞的小鼠比接受白色对照脂肪细胞的小鼠体重增加变少。与接受白色对照脂肪细胞的小鼠相比,接受HUMBLE或棕色对照脂肪细胞移植的小鼠在葡萄糖耐受性和胰岛素敏感性方面也有约30~35%的改善。

血清胰岛素没有统计学上的显著变化;但是,与接受白色对照脂肪细胞移植的小鼠相比,HUMBLE和棕色对照脂肪细胞组的循环甘油三酯浓度降低。移植HUMBLE或棕色对照脂肪细胞的小鼠在暗循环中也比接受白色对照脂肪细胞的小鼠消耗更多的氧气和产生更多的热量,而三者食物摄入量没有差异。与接受白色对照脂肪细胞移植的小鼠相比,HUMBLE或棕色对照脂肪细胞移植使小鼠在冷暴露时保持较高的核心体温。

研究人员还在6个独立的队列中观察到HUMBLE细胞移植的代谢表型的改善,每个队列有6到8只小鼠。值得注意的是,与单独移植基质胶的小鼠相比,白色对照脂肪细胞移植对体重、葡萄糖耐量、耗氧量或产热没有影响,这说明白色对照脂肪细胞的移植对代谢没有任何负面影响。

从机制上看,HUMBLE脂肪细胞中精氨酸/一氧化氮(NO)代谢的增加促进了红细胞中由S-亚硝基硫醇和亚硝酸盐携带的NO的产生,以激活内源性棕色脂肪,改善了受体动物的葡萄糖平衡。

这些数据共同证明,使用CRISPR-Cas9技术对人类白色脂肪细胞进行工程设计,可以得到棕色脂肪样表型,并可能为防治肥胖和糖尿病开辟基于细胞的治疗机会。
