Hepatology:更便宜的EEG系统也能诊断肝性脑病

脑电图(EEG)能够客观的诊断肝性脑病(HE),并根据其严重性进行分级,不过需要需要昂贵的设备。而近期开发出的一款低成本、易使用的EEG系统,就算神经生理学的经验有限,也能对该系统的使用游刃有余。研究者进行了一项研究,评估从标准EEG(Standard-EEG)系统和商业化、低成本的无线耳机(Light-EEG)所获取的EEG数据与肝硬化和不同程度HE患者的关系。研究共纳入72名进行临床评估的患者

脑电图(EEG)能够客观的诊断肝性脑病(HE),并根据其严重性进行分级,不过需要昂贵的设备。而近期开发出的一款低成本、易使用的EEG系统,就算神经生理学的经验有限,也能对该系统的使用游刃有余。研究者进行了一项研究,评估从标准EEG(Standard-EEG)系统和商业化、低成本的无线耳机(Light-EEG)所获取的EEG数据与肝硬化和不同程度HE患者的关系。

研究共纳入72名进行临床评估的患者,其中男性有58名,年龄61 ± 9岁。获取患者心理肝性脑病评分(PHES)、Standard-EEG和Light-EEG信息。

研究数据显示,从两种EEG系统(Standard-EEG和Light-EEG)获取的自动参数具有强关联。Bland分析和Altman分析显示,两种EEG系统提供的自动参数具有可比性,基于Standard-EEG和Light-EEG对患者进行正常或异常EEG的分类一致性良好(0.6< k <0.8)。此外,从Light-EEG获取的自动参数如平均优势频率(MDF)终末期肝病模型(MELD)评分显著相关(r=-0.39, p<0.05),此外还与禁食、静脉血氨水平(r=-0.41, p<0.01)、PHES性能(r=-0.49, p<0.001)相关。最后研究者还观察到,不同严重程度HE患者的Light-EEG自动参数具有显著差异。

研究结果表明,可以从低成本、商业化的无线耳机(Light-EEG)获取到可靠的可用于HE诊断和分级的EEG参数。无论是日常临床工作还是研究背景下,这种患者自主工具均可以得到广泛使用。
