“岩”是中医对癌症的描述,祖辈们很早就认识到“质硬如石”是恶性肿瘤的重要临床特征。然而,肿瘤细胞实际上并不硬,质硬是由于肿瘤微环境中的成纤维细胞激活分泌胶原所致。因此,成纤维细胞激活是恶性肿瘤的共同特性。

众所周知,肿瘤细胞犹如一颗“种子”,其生长离不开沃土,即肿瘤微环境。通过近年来的研究表明,成纤维细胞是肿瘤微环境中数量最丰富的固有间质细胞成分,在恶性肿瘤治疗耐受中起到了举足轻重的作用。

早期研究发现,成纤维细胞能促进肿瘤发生发展,提示其可能是良好的治疗靶点,然而近期的临床试验却发现针对成纤维细胞的治疗反而会加速癌症病人的病情发展。同时,既往研究也提示肿瘤相关成纤维细胞具有高度的异质性且缺乏有效分群的表面标记物,因此导致了身份识别困难严重,并阻碍了对不同亚群成纤维细胞功能的研究以及靶向治疗手段的研发。

宋尔卫团队率先采用术前化疗作为研究肿瘤微环境异质性的临床模型,有效规避了临床治疗手段对肿瘤生物学特性的影响。研究发现,未经治疗的术前穿刺标本中成纤维细胞数目与日后的术前化疗敏感性无关。但是,经过术前化疗后,耐药标本的成纤维细胞数目明显高于敏感标本。在体外实验中,研究人员将肿瘤标本中分离出来的成纤维细胞和肿瘤细胞进行共培养,结果发现,从耐药标本分离的成纤维细胞能诱导共培养的肿瘤细胞耐受化疗药物打击,而敏感标本分离的成纤维细胞则不能。研究进一步证实:在恶性肿瘤中存在不同亚群成纤维细胞发挥不同生物学功能的科学猜想。

此外,通过对比耐药和敏感标本成纤维细胞的基因表达谱的差异,该研究利用 CD10、GPR77这两个细胞膜蛋白鉴定出一个CD10+GPR77+成纤维细胞亚群,并发现这类型的成纤维细胞数目与乳腺癌肺癌病人的预后和化疗敏感性相关。同时,该研究提示,在治疗前通过患者肿瘤标本检测特异亚群成纤维的数目有望可以作为预测乳腺癌及肺癌病人预后和化疗敏感性的一项有效临床指标,能更好地指导临床治疗方案的选择。

为了最大程度地模拟病人的肿瘤微环境,研究人员还采用了病人来源异质瘤(PDX)的动物模型,将富含CD10+GPR77+ 成纤维细胞的人肿瘤标本直接移植到小鼠脂肪垫,进而使用GPR77膜蛋白的阻断抗体联合化疗药物进行治疗。实验结果显示,抗体阻断治疗能够显著增强移植瘤的化疗敏感性,该研究为靶向微环境逆转肿瘤耐药的治疗提供了新靶点,具有重要的研究价值。
