Ann Neurol:轻度认知障碍患者脑神经病理学病变

轻度认知障碍患者通常有多种神经病理学病变,尸检发现仅有不到四分之一的轻度认知障碍病例表现为单一性脑神经病变

近日,研究人员对轻度认知障碍临床诊断后的临床及神经病理结果进行研究。

研究数据来自于全美4个阿尔茨海默病研究中心的1337例正常或轻度认知障碍后的死亡患者尸检报告。

研究中位随访7.9年,其中874人诊断为轻度认知障碍。最终临床诊断数据如下:39.2%的死者诊断为轻度认知障碍,46.8%的死者诊断为痴呆,13.9%的死者意识正常。在非AD病理组中,与认知障碍相关的tau蛋白病(P<0.05)和脑动脉硬化的病变(P<0.001)较意识正常死者更为严重。在轻度认知障碍死者中,有55%存在超过一种以上的AD脑神经病变(ADNC),22%表现为单一的ADNC,22%表现为非阿尔茨海默病理。74%的轻度认知障碍死者不存在高度的AD脑神经病变、Lewy体病或海马硬化病变,但这些死者有多种的脑血管疾病。痴呆但无严重神经退行性病变的死者往往有脑血管病变且长时间没有进行认知障碍的检查。
