急性脑出血强化降压试验(INTERACT2研究)最新研究结果,1月26日,在线发表于《卒中》(Stroke)杂志上,确定了基线血糖、糖尿病和急性脑出血强化降压受试者临床结局的关系。


国际前瞻性、开放、盲法和随机对照INTERACT2研究,包括2839名自发性(<6小时)脑出血和收缩压升高患者,随机分入强化降压(目标收缩压<140mmHg)或基于指南(收缩压<180mmHg)进行血压管理。Logistic回归模型确定高血糖(>6.5mmol/L)与联合和独立90天较差结局死亡和主要残疾相关性。

共有2653例可用的患者数据,1348例(61%)有高血糖,292例(11%)有糖尿病。基线血糖和较差结局强烈和持续相关。调整基线变量后,血糖最高四分位(7.9-25.0mmol/L)水平与较差结局显著相关(调整的OR为1.35,95%CI1.01-1.80,Ptrend=0.015)。糖尿病同样可用预测较差结局(调整的OR为1.46,95%CI1.05-2.02,Ptrend=0.023),分别统计后,致残作用大于死亡。

研究提示,高血糖和糖尿病是轻中度脑出血患者较差结局主要的独立预测因素。研究数据支持指南推荐脑出血患者进行血糖控制。
