Blood:多发性骨髓瘤粒细胞髓系抑制细胞的免疫基因组学鉴定及特征

在骨髓瘤微环境中,从未成熟到成熟的中性粒细胞存在一个渐进的免疫抑制梯度。 CD11b+CD13+CD16+成熟中性粒细胞在表观遗传学上是不受控制的,它们在骨髓瘤微环境中的丰富程度具有预后意义。

粒细胞性髓样抑制细胞(G-MDSCs)促进多发性骨髓瘤(MM)的肿瘤生长和免疫抑制。然而,还不能准确的检测其表型,及用于临床转化。

本研究旨在提供基于多发性骨髓瘤预后意义、免疫抑制潜能和分子程序的G-MDSCs表型图谱。

通过多维流式细胞术检测了正常人和MM患者骨髓样本中预先建立的G-MDSCs表型,令人惊讶的是,研究人员发现CD11b+CD14-CD15+CD33+HLADR-细胞与MM患者中常见的嗜酸性粒细胞和中性粒细胞重叠。因此,研究人员依靠自动聚类来公正地识别肿瘤微环境中的所有粒细胞亚群:嗜碱性粒细胞、嗜酸性粒细胞、未成熟中性粒细胞、中性粒细胞和成熟中性粒细胞。

在267名新确诊的MM患者队列(GEM2012MENOS65试验)中,只有确诊时成熟中性粒细胞的占比与患者的预后显著相关,并且高成熟中性粒细胞/T细胞比率导致较差的无进展生存率(P<0.001)。

对每个中性粒细胞亚群进行FAC分选后,在存在成熟中性粒细胞的情况下,T细胞增殖显著降低(0.5倍,p=0.016),而被BCMAxCD3双特异性抗体激活的T细胞的细胞毒性随着成熟中性粒细胞的耗竭而显著增强(4倍;P=0.0007)。

最有趣的是,这三个亚群的RNAseq显示,由于染色质可及性的不同,与对照组相比,多发性骨髓瘤患者成熟中性粒细胞的G-MDSCs相关基因特异性上调。

综上所述,研究人员建立了明确的中性粒细胞亚群的临床意义、免疫抑制潜能和转录网络之间的相关性,首次提供了一套可准确监测MM中G-MDCS的最佳标志物(CD11b/CD13/CD16)。
