Environ Health Perspect:孕期较高的硒和锰水平保护婴儿免受未来高血压的影响

高血压(BP)是血管疾病(CVD)的主要危险因素,包括心脏病、中风、肾衰竭和阿尔茨海默氏病。它也非常普遍;每年在全世界造成780万人死亡。英国石油公司从童年到成年都在追踪,儿童血压升高与成年期高血压和心血管疾病有关。因此,确定儿童高血压的可改变的危险因素可能为心血管疾病的原始预防铺平道路。高血压的定义为收缩压超过140毫米汞柱或舒张压超过90毫米汞柱。

近日在Environmental Health Perspectives杂志上约翰霍普金斯大学彭博公共卫生学院的研究人员领导的一项研究中,在母亲怀孕期间接触到较高水平的微量矿物质锰和硒的儿童,在童年时患高血压的风险较低。研究人员还发现,当母亲血液中的镉(一种有毒的重金属)水平较高时,锰与儿童血压有更强的反向关系--暗示锰降低血压的部分原因是对抗镉的血压升高效应。

硒(Se)和锰(Mn)是人体必需的微量元素一些研究表明Se和Mn可能对高血压和CVD有保护作用铅(Pb)、汞(Hg)和镉(Cd)是一种毒性很高的金属,即使在微量水平上也会对健康造成不利影响。Mn和Se具有抗氧化的特性,并存在于各种食物中,包括坚果和谷物、多叶蔬菜、鱼和贝类。

研究人员分析了波士顿地区近1200名在2002年至2013年期间分娩的妇女的血液样本中的有毒金属和微量元素的水平。他们发现,母亲血液中较高的Se或Mn水平与她们的孩子在3到15年后的门诊中较低的血压读数有关。

母亲的硒水平和其后代在儿童时期的血压降低之间的联系。母亲的硒水平每增加一倍,儿童的收缩压就会平均降低6.23点。锰与血压有类似的关系,表现不明显,暴露量增加一倍,收缩压平均降低2.62点。

结果表明,母亲在怀孕期间饮食中的硒和锰的健康水平可能会保护她们的孩子免受高血压的影响,强调了子宫内的营养和环境暴露对孩子心血管健康的重要性。用于分析的数据集涵盖了来自波士顿出生队列研究的1194对母子。孩子们的血压读数是在3-15岁之间进行的。大多数母亲是黑人(61%)或西班牙裔(20%)。

Cd本身与儿童血压没有关系,但研究人员发现,当母体血液中的Cd含量较高时,Mn与儿童血压之间的反比关系明显更强。这一发现表明,Mn可以专门保护Cd的高血压促进作用,甚至可能掩盖正常人群中镉的高血压促进作用。人们通常认为接触Cd等重金属只发生在职业环境中,但事实上这些金属就在我们身边--例如,普通香烟的烟雾中就有Cd。他们将儿童的血压读数与他们母亲血液中的有毒金属和微量元素的水平进行了比较,测量了有毒金属Pb、Hg和Cd。

研究发现孕期接触香烟烟雾的儿童,与高锰暴露相比,同时接触低锰的儿童在儿童时期可能更容易出现更高的血压。在Cd含量较高的母亲中,Mn对儿童SBP的保护作用更强这强调了Mn与Cd的明显联系。对于怀孕期间吸烟的母亲来说,优化锰水平可能会保护他们的孩子免于患上高BP和未来的心血管疾病。

希望未来的研究证实能宫内Mn、Se与儿童血压及Mn、Cd对儿童收缩压的交互作用。并能了解到这些关联背后的分子和代谢途径,如何将这些发现转化为临床和公共卫生实践,预防儿童血压过高。
