肠道菌群参与了尿毒症溶质的代谢。然而,微生物对于CKD患者尿毒症溶质潴留的确切影响尚不清楚。为了澄清这一点,近日,肾脏病领域权威杂志kidney international上发表了一篇研究文章,研究人员在无菌或无特定病原体(SPF)的条件下比较了腺嘌呤诱导的肾功能衰竭和对照小鼠,使用毛细管电泳飞行时间质谱为基础的方法检测了血浆、粪便和尿液中的代谢物。

无菌状态下的肾功能衰竭小鼠血浆代谢产物显著改变。在183个检测的溶质中,血浆中有11种溶质,包括主要的尿毒症毒素,在无菌肾功能衰竭小鼠要比SPF肾功能衰竭小鼠显著降低。这11种溶质被认为是微生物产生的尿毒症溶质,包括硫酸吲哚酚、p-硫酸对甲酚、苯基硫酸、胆酸钠、二甲基马尿酸、γ-胍基丁酸、戊二酸、2- 羟基戊酸乙酯、三甲胺N-氧化物和phenaceturate。

代谢组学分析表明,这些物质根据他们的来源可以被归为三类:完全来自微生物(硫酸吲哚酚、p-硫酸对甲酚)、来自宿主和微生物(二甲基马尿酸)以及来自微生物和膳食成分(三甲胺N-氧化物)。此外,无菌性小鼠肾功能衰竭导致结肠短链脂肪酸消失,肠道氨基酸利用率降低,并且肾损伤更严重相比于SPF小鼠。微生物产生的短链脂肪酸和足够的氨基酸利用效率可能具有肾保护作用,并且这些因素的消失可能加剧无菌肾功能衰竭小鼠的肾损伤。

因此,微生物有助于产生有害的尿毒症溶质,但反过来,没有微生物的生长对CKD进展有不利影响。
