盘点:“低脂饮食”真的很健康吗?

低脂饮食究竟是否真的健康?尚存在争议。

【1】低脂饮食可延长ER-乳腺癌患者生存

研究纳入2437 I期到IIIA期乳腺癌患者,其中,1579例雌激素受体阳性患者,478例雌激素受体阴性患者,362例雌雄激素受体均阴性患者,年龄48-79岁之间,并接受了正规的治疗。准入标准包括有或没有接受放疗的一期手术患者,并进行了系统的治疗(雌激素受体阴性的患者接受化疗,雌激素受体阳性患者接受他莫昔芬联合化疗)。研究启动时,所有参与者均有超过20%的高脂饮食摄入量。

研究者随机分配两组:饮食干预组975例,对照组1462例。饮食干预组要求患者5年内降低脂肪摄入并保证营养均衡,患者需要每两周参加一次注册营养师咨询会,每3个月与营养师联系。干预组接受降脂目标,记录各自的脂肪摄入,研究者会不定时电话检测患者的脂肪摄入情况。5年后,干预组降低了9.2%脂肪摄入,比对照组降低了6磅。

结果显示,低脂饮食干预组比对照组降低了24%的复发风险。总体上,低脂饮食干预组与对照组平均生存无显著差别:干预组死亡率较低 (13.6% vs. 17%; HR=0.94; 95% CI, 0.76-1.2),但没有统计学意义。亚组分析1,低脂饮食干预组ER阴性患者比对照组平均生存明显延长:(13.6 vs 11.7年,HR=0.64; 95% CI, 0.41-0.99),且意义显著。亚组分析2,低脂饮食干预组ER/PR均阴性患者比对照组平均生存时间更长:(14 vs 11.7年,HR=0.46; 95% CI, 0.27-0.78)。ER阴性和ER/PR均阴性的患者在随机分组后10年或干预结束后5年生存获益达高峰,随机分组后15-20年减弱。(文章详见——SABCS 2014:低脂饮食可延长ER-乳腺癌患者生存

【2】高纤维低脂肪饮食能够降低结肠癌风险


研究招募了非裔美国人与非洲本土居民各20人,让他们在严格的控制条件下进行了该试验,即在2周的时间内互换了饮食的方式。分别采用了肠镜检测和常用分子标志物进行检测。

在试验开始前,约半数的美国受试者均有肠道息肉——一种致癌的潜在风险,而非洲受试者无此症状。结果显示,2周的试验过后,美国组的结肠内炎症现象明显减弱,肿瘤风险相关标志物的表达也减弱。而非洲受试者在食用了2周的美国高脂高蛋白饮食后,肿瘤风险标志物的表达均出现了显著上调。

【3】低脂肪饮食有助于绝经后女性避免致命性乳腺癌

研究者对48835绝经后女性进行了随访分析。这些女性年龄50-79岁,事先没有乳腺癌,具有正常乳房X线照片和正常的脂肪摄入。其中,19541名女性被置于一个低脂肪饮食与营养学家带领的小组,脂肪的摄入量减少能量的20%,增加水果,蔬菜和谷物的消耗。另外29294名女性遵循自己平时的饮食模式。

经过大约8年的低脂肪饮食,1767名女性被诊断患有乳腺癌。研究人员发现,饮食组的乳腺癌诊断后的总生存期要高:82% vs 78%。研究人员说,这种减少在某种程度上是由于,乳腺癌诊断后有更好的生存率。

研究人员还报告说,大多数乳腺癌的特点-包括大小、淋巴结状态、预后差的分布、三阴性癌症和HER2阳性的癌症-在两组女性中是相似的。但在饮食组有更少的孕激素受体阴性肿瘤(28.4% vs 33%)。此外,研究人员还指出,在饮食组中还有较低的心血管疾病死亡率。(文章详见——AACR 2016:低脂肪饮食有助于绝经后女性避免致命性乳腺癌

【4】想减肥?低脂饮食效果更好


研究者对19名肥胖患者进行了研究。实验分两个阶段进行:第一阶段,限制30%的参与者饮食中的碳水化合物含量,脂肪摄入量不变;第二阶段,限制另外30%参与者饮食中脂肪摄入量的含量,碳水化合物摄入量不变。通过测量参与者每日脂肪摄入量和脂肪氧化率的差异,由此计算出参与者机体脂肪的减少量。

研究人员通过比较低脂饮食和低碳水化合物饮食,他们发现,低脂饮食更能有效减少体内脂肪。(文章详见——Cell Metabolism:想减肥?低脂饮食效果更好

但是Lancet子刊上就“低脂饮食能否减肥”这个问题,提出了不同的意见。不仅如此,还有机构质疑低脂饮食的健康性。

【5】低脂肪饮食干预真的可以减肥吗?

研究人员做了一项系统性回顾和随机效应荟萃分析,比较了低脂肪和高脂肪的饮食干预的长期效果(≥1年)。研究人员确定了3517篇引文,53项研究符合纳入标准,其中包括68128名参与者(69种比较)。

在减肥试验中,低碳水化合物干预比低脂干预能显著减少体重(18种比较;WMD 1.15kg[95%Cl 0.52至1.79];I2=10%)。低脂肪的干预措施与其他高脂肪体重减轻干预措施相比并没有显示出体重变化的差异(19种比较,WMD 0.36千克[-0.66至1.37;I2=82%),只有与平时饮食进行比较,才能导致体重显著下降(8种比较;-5.41kg[-7.29至-3.54];I2=68%)。

非减肥试验和体重维持试验中,对于没有低碳水化合物进行比较的试验中,结果表明,低脂肪与高脂肪的干预对减肥有类似的效果,而低脂肪干预只有当与平时的饮食相比才能导致更大体重损失。在减肥试验中,随访过程中当组间差异超过5%热量摄入时(18种比较;WMD 1.04千克[95 %Cl 0.06至2.03];I2=78%),并且当在后续随访中两种干预之间血清甘油三酯的差至少为0.06mmol/L时(17种比较;1.38kg[0.50至2.25];I2= 62%),高脂肪的减肥干预比低脂肪的干预导致显著更大的体重损失。

结果表明,低脂肪饮食干预对体重的长期效果取决于对照组干预的强度。当与类似强度的饮食干预相比时,RCTs的证据并不支持低脂肪饮食比其他饮食干预对长期减肥更有效果。(文章详见——Lancet Dia & Endo:低脂肪饮食干预真的可以减肥吗?

【6】低脂食品真的健康吗?

一项囊括50000名女性,长达8年的研究表明:大约有一半的女性选择低脂饮食,但这一饮食方式并不能降低她们患乳腺癌,结肠癌以及心脏病的风险。此外,她们的体重也没有明显的减低。

最近发表在《JAMA》杂志上的一篇文章表明糖分,而非脂肪,是问题的关键。

在一项最近的研究中,作者们通过调查问卷的方式分析了126000名志愿者三十年来的饮食习惯。之后,他们尝试将这些志愿者的饮食中饱和脂肪酸(主要存在于肉类与奶类食品中)提供的5%的能量替换为别的营养类型:1、简单碳水化合物,如葡萄糖或加工谷类食品;2、单不饱和脂肪酸,比如牛油果或橄榄油;3、多聚不饱和脂肪酸,比如鱼类与坚果。

理所当然地,第一种替换方式没有起到任何的促健康效应。

但第二种与第三种方式似乎能够起到一定的促进健康的效果。第二种替换方式能够有效降低27%的死亡率,心脏病、癌症与神经退行性疾病的发病风险也发生了明显的降低。

第三种替换方式能够降低13%的死亡率,神经退行性疾病引发的死亡风险也有明显降低。

这一研究结果与近期一些别的研究结论一致,即某些类型的脂肪是由促进健康的作用的,只要我们适当地摄入。(文章详见——JAMA INTER MED:低脂食品真的健康吗?

【7】某机构质疑膳食指南,认为“低脂饮食”会带来毁灭性的健康后果

英国的某健康慈善机构警告说,要求人们遵循低脂饮食将会带来“灾难性的健康后果”。

他们呼吁其对目前的膳食指南“大修”。他们说低脂饮食不能解决英国的肥胖危机,使人发胖的不是脂肪,而是零食!

他们呼吁全方面的饮食,包括肉类、鱼和奶制品,以及高脂肪、健康的食物如鳄梨等,并说,“吃脂肪不会使你发胖!”

他们同时也认为,饱和脂肪不会导致心脏病,而脂肪乳制品,包括牛奶、酸奶和奶酪,实际上可以保护心脏。

标有“低脂肪”、“精简”、“低胆固醇”或“证明能降低胆固醇”的加工食品应该避免,2型糖尿病患者应该多吃富含脂肪的饮食,而不是一个基于碳水化合物的饮食。

报告还说,糖是应该避免的,人们应该停止计算卡路里,认为锻炼可以帮助你摆脱不良的饮食也只是一个神话。相反,饮食中的碳水化合物含量很低,但健康的脂肪含量很高,且是一种能防止体重增加和帮助减肥的有效、安全的方法,同时还可以降低心脏病风险。

报告还说:“吃富含脂肪的乳制品,如奶酪、牛奶和酸奶,实际上可以降低肥胖的风险。”

“最自然、最营养的食物如肉、鱼、蛋、乳制品、坚果、种子、橄榄和鳄梨等都含有饱和脂肪。继续妖魔化无所不在的天然脂肪,将会促使人们远离高营养、促进健康的食品。”

报告的作者还指出,食品科学也受到商业的影响,被“损坏”了。

国家肥胖论坛主席David Haslam教授说:“作为一名每天治疗病人的医生,我很快意识到目前的膳食指南几乎是在说明,高碳水化合物、低脂肪饮食是万能药,这是具有严重缺陷的。”

“尽管有着指南的实施,但是当前的肥胖情况仍比以前严重,这些努力并没有减缓肥胖的脚步。”

公共卫生合作的创始成员,心脏病学家Aseem Malhotra博士说:“当前推荐低脂饮食的膳食指南或许是现代医学史上最大的错误,将会给公共卫生造成毁灭性的后果。”

“可悲的是,这种无益的指南建议仍在继续延续。目前英格兰公共卫生的膳食平衡指南在我看来更像是一个定时炸弹,而不是有利于代谢健康膳食模式。我们必须尽快改变这些针对公众的推荐信息,以扭转肥胖和2型糖尿病风险。”

“吃肥肉也能减肥。不要怕胖,脂肪是你的朋友。现在是脂肪回归的时刻了。”

亚伯丁罗伯特戈登大学的Iain Broom教授说:“继续推荐高碳水化合物、低脂低卡路里饮食作为“健康饮食”将是致命的缺陷。”

“我们的人口在近40年来一直处于一个不受控制的全球实验状态,这已经是一个严重错误。”

皇家医师学院肥胖特别顾问John Wass教授说:“虽然有充分的证据表明,饱和脂肪会增加胆固醇。”

他补充说:“但是我们需要的是平衡的饮食、有规律的运动和正常健康的体重。选择性引用研究将会产生误导公众的风险。”

公共卫生学院的Simon Capewell博士说:“我们完全支持英格兰公共卫生提出的新的健康饮食指南。他们的建议是以科学证据为基础,我们相信科学。并且它没有受到工业的影响。”

“相比之下,国家肥胖论坛的这份报告不是同行审查的,也不知道这是谁写的,如何写出来的,这些都是疑点,不能对其盲目相信。”

英国心脏基金会(BHF)医学副总监Mike Knapton博士说:“虽然这份报告里的所有想法和意见没有提供稳健和全面的证据支持,但是作为英国最大的心脏研究慈善机构,我们将重视该问题,并进行相关调查研究。”

“不过国家的肥胖流行并不是不良饮食指南导致的,这也是我们不同意他们的一方面原因。”

PHE的首席营养师Alison Tedstone博士说:“在所有的证据面前,呼吁人们多吃脂肪,减少碳水化合物和卡路里的摄入是不负责任的。与他们不同的是,我们是独立专家审查所有可用的证据,往往是数以千计的科学论文,并尽一切努力,经过全面的协商,以确保没有偏见的存在。”
