NEJM:低温治疗癫痫持续状态的利与弊

在多中心临床试验中,研究者们随机分配270例惊厥性癫痫持续状态的危重症患者,这些受试者除了接受标准治疗或单独的标准治疗以外,还接受机械通气至低体温(32至34℃维持24小时);最终有 268例患者纳入分析。

此项研究的主要次要结果是在90天的死亡率,进展到脑电图(EEG)确认的癫痫持续状态,第1天的顽固性癫痫持续状态,“超级顽固”状态癫痫持续(抗全身麻醉)和在90天的功能性后遗症。

研究结果显示:138例的低温组受试者中有67例(49%)GOS评分为5分,130例对照组受试者中有56例(43%)GOS评分为5分(调整后的OR值为1.22; 95%CI 0.75〜1.99 ; P = 0.43)。

第一天低温组的EEG确诊癫痫持续状态的发生率低于对照组(11%vs. 22%;OR值,0.40; 95%CI,0.20至0.79; P = 0.009),但是其他次要结局组间无显著性差异。低温组的不良事件较对照组更频繁。

此项研究最后得出结论,在惊厥性癫痫持续状态患者中使用诱发低温联合标准治疗方法较单独的标准治疗方法相比,其与更佳的90天结局并无显著相关性。
