JCMR:扩散加权心血管磁共振成像在鉴别急性与非急性深静脉血栓的价值

鉴别急性与非急性深静脉血栓是非常重要的,本研究旨在验证读出分割扩散加权(DW)心血管磁共振(CMR)在鉴别急性和非急性深静脉血栓(DVT)的价值。

方法:本前瞻性研究收集了85例DVT的患者,并行读出分割DW CMR检查。急性DVT定义为血栓形成时间≤14天(n=55),非急性DVT为>14天(n = 30)。利用4分量表(0~3为差-优)在b = 0、b = 800和表观扩散系数(ADC)图像上对DVT显示情况进行评价。利用感兴趣区(ROI)方法对 DW CMR参数进行测量。利用Mann Whitney检验比较急性DVT和非急性DVT的相对信号强度(rSI)和ADC值。计算rSI和ADC值的敏感性和特异性。

ADC图对于血栓的评分要明显高于b = 0和 b = 800 图像(P < 0.05)。在急性和非急性DVT间显示效果无统计学差异。

结果为,ADC图血栓显示评分要显着高于b = 0、b = 800图像s (2.7 ± 0.5, 2.5 ± 0.6、2.4 ± 0.6, P<0.05)。急性DVT平均ADC值要明显高于非急性DVT DVT (0.56 ± 0.17 × 10-3 vs. 0.22 ± 0.12 × 10-3 mm2/s, P<0.001)。以0.32 × 10-3 mm2/s作为最佳截断值,ADC鉴别急性DVT和非急性DVT的敏感性和特异性分别为93%、90%。rSI在b = 0图像上分别为73%、60%, 在 b = 800图像上分别为75%、 63%。

本研究表明,读出分割扩散加权CMR得到的ADC值有助于鉴别急性和非急性DVT。
