JNCI:糖尿病和种族差异与肝细胞性肝癌的发病风险相关

肝细胞性肝癌(Hepatocellular carcinoma,HCC)是成人肝癌最常见的类型,也是死亡风险最高的癌症之一,五年生存率仅16%。研究发现,肝细胞性肝癌的发病率在亚裔、西班牙裔和非裔美国人中存在种族差异。此外,糖尿病也是肝细胞性肝癌可能的危险因素,而近年快速上升的糖尿病发病率可能引起了肝细胞性肝癌发病率的上升。糖尿病在西班牙裔和非裔美国人中的发病率尤其高,可能与肝细胞性肝癌发病率的种族差异有关。目前,来自南加州大学的研究者首次研究了不同的种族间,糖尿病与肝细胞性肝癌的发病风险的相关性。

本文是多种族队列(Multiethnic Cohort,MEC)研究的一部分,MEC研究是一项还在进行中的基于人群的前瞻性队列研究,目的是研究癌症和其他慢性疾病的饮食、环境和基因方面的危险因素。该队列建立于1993~1996年间,纳入了215000名来自夏威夷和加州的观察对象,包括非裔、夏威夷原住民、日裔、拉丁裔及高加索裔。人口学资料、生活方式、糖尿病患病情况等资料来自患者自评问卷。肝细胞性肝癌的诊断来自当地的肿瘤登记记录。

随访15.7年后,168679名高危的观察对象中有470名被诊断为肝细胞性肝癌,其中53名白人、78名非裔,31名夏威夷原住民,150名日裔及158名拉丁裔。以白人为参照,其他种族患HCC的相对危险度(RR)分别是拉丁裔2.73(95% CI=1.52~3.07),夏威夷原住民2.48(95% CI = 1.59~3.87),非裔2.16(95% CI = 1.52~3.07),日裔2.05(95% CI = 1.50~2.81)。在各种族糖尿病均与HCC发生有关:RR拉丁裔= 3.36(95% CI = 2.41~4.70),RR夏威夷原住民= 2.50(95% CI = 1.11~5.64),RR日裔= 2.34(95% CI = 1.60~3.41), RR白人= 2.15(95% CI = 0.95~4.90,RR非裔=2.02(95% CI = 1.17~3.48)。最后,研究者根据人群归因危险度百分比估计可通过避免糖尿病而降低患HCC的人数比例: 27%的拉丁裔HCC患者、18%的夏威夷原住民HCC患者、13%的非裔HCC患者、12%的日裔HCC患者及6%的白人HCC患者。
