卢光琇教授:大数据时代的临床遗传学策略

大数据在临床遗传学最经典的应用就是基因组学,应用下一代测序技术(NGS)产生的海量数据与基因序列分析技术一起,让发现疾病的过程变得更快、更容易和更便宜。目前,其主要应用于遗传病病因分析,产前检测和植入前胚胎遗传学分析。通过对遗传性疾病相关基因检测、全基因组外显子测序分析以及全基因组序列分析对遗传病病因进行分析,并应用于产前诊断、胚胎植入前遗传学筛查(PGS)和胚胎植入前遗传学诊断(PGD)。

早在二十世纪八十年代,卢教授就率先在国内建立了一套完整的人类辅助生殖技术体系,并最早在国内开展不孕与遗传门诊,是著名的生殖医学与医学遗传学家,也是我国人类生殖工程创始人之一。中信湘雅生殖与遗传专科医院的生殖中心也是国际上接受试管婴儿治疗人数最多、妊娠率最高的生殖中心之一。因此卢教授有丰富的临床遗传咨询经验,她也在会上与大家分享了一些临床实例,例如采用NGS分析复杂疾病表型和利用NGS提高诊断能力,同时又举例说明NGS技术的不足之处,如大于20bp的插入、缺失和重复突变以及三核苷酸重复拷贝数变异检测灵敏度低,并对NGS得到的大量数据中疑似致病位点的临床意义需持谨慎态度。

最后,卢教授指出,NGS帮助遗传咨询医师扩大了遗传性疾病检测的范围,提高了检测效率,同时临床遗传咨询医生需要了解各类基因检测的缺陷和局限性,重视临床指征和家系分析,关注研究进展,从而帮助患者从新的技术和研究中最大程度获益。
