卵巢癌是一种致命的疾病,是出了名的难诊断治疗的癌症。加州大学圣地亚哥医学院以及Moores癌症中心科学家鉴定出了卵巢癌细胞的6个mRNA亚基(信使RNA),使早期卵巢癌的诊断成为可能。更重要的是,在这些mRNA亚基中,有些能编码针对新疗法的特殊蛋白质。这项研究成果5月25日发表在Proceedings of the National Academy of Sciences上。

“过去很多研究旨在利用DNA检测癌症”,该研究的第一作者(加州大学圣地亚哥遗传组医学院信息生物学专家以及项目科学家) Christian Barrett博士说,“我们很好奇,如果我们开发一种基于肿瘤特异性mRNA的卵巢癌检测方法来代替DNA检测的方法,会是怎样的结果?这种肿瘤特异性mRNA从肿瘤细胞扩散到子宫颈,且可以通过常规的巴氏试验收集”。

DNA与mRNA有什么联系和区别呢?DNA携带了生命所必须的所有指令,它包含的不仅仅是编译蛋白质的序列,还包含其他序列。而mRNA是DNA的互补序列,携带了蛋白质的所有信息。细胞核的DNA把遗传物质传递给mRNA,由mRNA转移都细胞质,在细胞质中各种细胞器可以识别mRNA中的信息,同时合成蛋白质。作者表明,利用肿瘤mRNA来诊断癌症优于DNA检测癌症的理由如下:一个癌症细胞可能只有一个或几个拷贝的DNA突变区,但可能有数百到数千拷贝的mRNA突变区。

为了确定是否可以通过mRNA从正常细胞中区分出卵巢癌细胞,研究人员开发了一个特定的生物信息学算法,并利用这个算法来挖掘两个公共遗传信息数据库——癌症基因图谱(TCGA)和基因组织表达程序(GTEx),这两个数据库都是由美国国立卫生研究所发起。TCGA编目了500个肿瘤的DNA和RNA,涵盖了许多种癌症类型,而GTEx是正常组织样本的DNA和RNA的数据库。基于此,研究人员可以分析295个卵巢肿瘤以及1839个正常组织样本的mRNA序列。

利用生物信息学的方法,研究人员确定了6种mRNA亚基分子,这些肿瘤特异性mRNA亚基的鉴定,对卵巢癌的早期诊断治疗有很大的帮助。同时他们用定量RT-PCR来验证其研究结果,用该技术来检测实验室培养的细胞中的卵巢癌mRNA分子。

除了在诊断方面的潜力,本研究中发现的mRNA亚基分子中个,有些还可以作为新的治疗靶点。科学家预测这些mRNA亚基可以翻译一些含有特殊氨基酸的蛋白质,这可使之成为一些特殊疗法的靶点,比如单克隆抗体、基于T细胞的疫苗等。更重要的是,卵巢癌特异性mRNA亚基本身就可以作为新的治疗靶点。

“我们是在实验室里研究一系列卵巢癌细胞得出的该结果,我们需要进行临床试验才能确定患有卵巢癌的妇女身上含有这种mRNA亚基分子,而未患卵巢癌的妇女身上没有这种mRNA亚基分子。”本研究的共同作者Cheryl Saenz医学博士(专门从事妇科癌症治疗的生殖医学临床专家)说,

本研究的作者承认他们的方法存在一些不足之处,其中包括mRNA亚基分子检测技术的不足,缺乏正常卵巢与输卵管的对照样本,肿瘤细胞向子宫颈扩散后基因表达可能与原始肿瘤不同。

不过,基于这些可喜的初步结果,作者建议将他们识别肿瘤特异性mRNA亚基的方法扩大到研究其他30个已知足够多RNA序列的癌症类型中。这6种mRNA亚型的发现将会有效的帮助广大女性进行早期的卵巢癌排查和诊断。同时也给卵巢癌治疗带来了新的方案

 1. mRNA诊断早期肝癌

mRNA诊断早期肝癌的研究也取得了进展。日本鸟取大学三岛典正先生和汐田刚史先生早些年成功开发了检测血清中hTERT的mRNA的高灵敏度定量方法,hTERT是和癌的发生、进展有关的端粒酶的催化单位。他们在对多个医疗机构的临床样品血清中的hTERT的mRNA进行评估的过程中,发现在肝细胞癌的诊断上该方法的灵敏度和特异性均优于其他肿瘤标记物,该成果发表在日本肝脏学会总会上。

 2. mRNA诊断早期宫颈癌

人半翼基因(hWAPL)是宫颈癌特异性高表达基因,检测hWAPL基因mRNA表达水平对宫颈癌早期诊断有重要的意义。几年前郑州大学第二附属医院妇产科研究人员使用高危型人乳头瘤病毒(HPV)检测试剂盒以及荧光定量PCR检测100例慢性宫颈炎、宫颈上皮肉瘤样变(CIN)和宫颈瘤患者的hWAPL基因mRNA和高危型HPV。结果显示高危型HPV检出率与宫颈癌发生密切相关,hWAPL基因mRNA表达与宫颈癌变直接相关,这有助于宫颈癌的早期诊断。该研究结果于2007年发布在《国外医学妇产科学分册》上。

 3.mRNA诊断膀胱癌

用RT-PCR检测尿液中survivin基因(该基因作为肿瘤诊断、判断预后和治疗的靶点)的mRNA,在国内外相关研究较多。与病理检查相比,mRNA诊断膀胱癌有较高的敏感度和特异度,可作为膀胱镜检查的主要辅助手段之一,用于膀胱癌的筛查及术后监测。

4.mRNA诊断直肠癌区域淋巴结微转移

准确判断新区域淋巴结有无肿瘤细胞早期转移,对其临床诊断、治疗及预后判断具有十分重要的意义。北京大学深圳医院的研究人员通过病理组织学及RT-PCR扩增K19 mRNA,对19例直肠癌患者的癌组织标本和51个淋巴结进行检测,结果表明19例直肠癌患者癌组织均有K19mRNA表达,51个淋巴结中组织学阳性16个(31.4%),而RT-PCR阳性21个(41.2%)。从而研究人员认为mRNA检测直肠癌区域淋巴结为转移是一种灵敏、特异的检测方法,优于常规病理组织学方法。该研究结果于2008年发表在Modern Oncology上。

5.mRNA诊断胃癌淋巴结微转移

研究胃癌微转移,有助于对恶心肿瘤进行更加全面的认识和更加合理的分期,为治疗提供帮助。应用实时荧光定量PCR的方法定量检测胃癌淋巴结CK18mRNA的表达水平,结果发现胃良性病变淋巴结CK18mRNA表达全部为阴性,而胃癌组织CK18mRNA表达全部为阳性,说明CK18mRNA的表达可作为胃癌淋巴结微转移的标志。该研究结果在2009年《全国临床免疫检验研讨会暨第六届全国临床免疫学术会议论文汇编》中有报道。

 小结

癌症早期诊断是一种专门针对癌症早期患者的诊疗方法,其目的在于早发现早治疗,从而减轻患者痛苦和精神、经济负担。争取通过癌症早期诊断治疗让癌症患者早日康复。基于mRNA分析诊断的发展将是分子医学领域的一场革命,它不仅提供了一种新的检测方法,并且具有高灵敏度、高特异性等特点。
