Oncotype DX复发评分(RS)、芯片50(PAM50)复发风险(ROR)的Prosigna预测分析、EndoPredict (EP)和乳腺癌指数(BCI)常用于临床评估接受内分泌治疗的患者远处

Oncotype DX复发评分(RS)、芯片50(PAM50)复发风险(ROR)的Prosigna预测分析、EndoPredict (EP)和乳腺癌指数(BCI)常用于临床评估接受内分泌治疗的患者远处转移的风险。可惜的是,这些评估方法的结果存在一定差异。该研究旨在确定驱动测试并导致这些差异的分子特征。

在ATAC试验的他莫昔芬或阿那曲唑治疗臂收集的TransATAC样本中分析RS、ROR、EP和BCI。纳入ER阳性/HER2阴性的未进行化疗的并可进行这四项检查的病例(n=785)。来自RS的雌激素、增殖、侵袭和HER2模块评分用于表征各自的分子特征。

除了RS vs ROR(ρ=0.32)和RS vs BCI(ρ=0.35)之外,这四种分子评分之间为中等至强相关(ρ= 0.63-0.74)。RS与其雌激素模块呈负相关(ρ=-0.79),与增殖模块呈正相关(ρ=0.36)。RS的增殖模块解释了72.5%的ROR变异,而雌激素模块仅解释了0.6%。EP和BCI的大部分变异是由增殖模块引起的(分别为50.0%和54.3%),仅小部分是由雌激素模块引起的(分别为20.2%和2.7%)。

与通常的理解相反,RS更多的是由雌激素相关的特征决定的,受增殖标志物的决定较小。但EP、BCI,尤其是ROR的评分主要是由增殖特征决定的。该结果有助于解释这些检测的预后性能上的差异。
