近日,发表在《美国心脏病学会杂志》(JACC)上的一篇综述文章中提出,为了促进血管健康,以蔬菜、水果、坚果、种子、豆类、全谷类和特级初榨橄榄油为基础的Pesco-Mediterranean饮食是最佳选择,同时辅以少量的鱼/海鲜和发酵乳制品。文章作者,密苏里州堪萨斯城圣卢克中美洲心脏研究所的James H. O’Keefe博士表示,这个饮食模式是为了平衡植物性饮食和动物性饮食的利弊。

如今,许多人过度消费动物性产品,通常是富含饱和脂肪和化学添加剂的加工肉类;或者严格的素食主义会导致营养缺乏,容易引发骨质疏松、肌萎缩和贫血。一个合乎逻辑的折中方案是食用富含植物的食物,同时以鱼类/海产品作为动物性食物的主要来源。

传统的地中海饮食(traditional Mediterranean diet)已经得到了《2015-2020年美国居民膳食指南》和《2019 ACC(美国心脏病学学会)/AHA(美国心脏学会)心血管疾病一级预防指南》的认可。O’Keefe博士介绍道,Pesco-Mediterranean饮食是这一概念的演变,并增加了细微差别,即鱼类/海鲜是首选的动物性蛋白。该综述的其他关键方面,是对无热量饮料的强调——主要是水,但也有茶和咖啡——以及间歇性禁食。他敦促说,除了食物的种类外,重要的是要考虑何时食用,科学也开始围绕这一点发展。

到目前为止,只有两个大型随机试验评估了地中海饮食对难治性CVD事件的影响:1997年对MI幸存者进行的Lyon Diet Heart研究和2013年一级预防的PREDIMED研究。

对于限时饮食(time-restricted eating),数据更为初步,不过,他们认为这种方法给健康带来了很多好处。Saeed认为这是另一个原因:不仅仅是在小学时看到的金字塔上整天吃素,而是在限定时间进食。

作者指出,关于这一主题的研究主要基于动物模型和观察人类研究。最受欢迎的限时饮食形式包括吃两餐而不是三餐,并压缩卡路里消耗窗口。目前还没有针对性的研究来评估最佳时间窗,但16:8的禁食与进食比例是最受欢迎的。

该文章的其他部分讨论了有关特级初榨橄榄油、Ω-3脂肪酸、坚果、豆类、全谷物、乳制品和鸡蛋的文献。酒精只是过眼云烟。作者建议,如果饮酒,建议饮用干红葡萄酒,最理想的饮用量是女性一杯(≤6盎司),男性每天一至两杯(6~12盎司)随餐饮用。

鱼类是“配菜”?

O’Keefe表示,他对营养与心血管健康的关系有着长期的兴趣,然而在他的医学培训中却没有这个话题。他认为应该改变这种情况,因为饮食对健康有深远的影响。这篇综述中所描述的Pesco-Mediterranean和间歇性禁食源自传统的饮食模式,这并不意味着吃意大利面和披萨,喝很多红酒。

丹佛市国立犹太医学中心Andrew Freeman博士认为地中海饮食是朝着正确方向迈出的一步,因为它主要是以植物为基础的。不过,需要注意的是,大多数美国人认为的地中海菜肴,如羊肉和羊奶饼,并不是文章所说的主要食材。当素食主义者以天然食物和植物为中心时,仍然有空间。

作为美国心脏病学院营养与生活方式工作组前任主席,Freeman补充道,第二,尽管吃鱼可能比吃牛肉或鸡肉好,但如果我们都按照推荐的量吃鱼,世界上就没有鱼了。现在即使是最野生和最偏远的捕获鱼类仍然富含[多氯联苯]、二恶英、重金属、有机磷酸盐和如今的微塑料。所以这类食物的供应尤其受到污染。而且由于鱼类处于食物链的高端,所有这些毒素都会积累起来。因此,Freeman建议,如果要吃动物产品,鱼比其他肉类更受欢迎,但应更多地被视为“配菜”而不是“主菜”。

他同意,限制一天的进食时间也很有意义。也就是说,这样做的目的是提醒人们不要在那些时期狼吞虎咽地吃垃圾食品和动物性食品。

O’Keefe博士同样认为,严格的纯素食主义者很难做到正确并保持健康。他建议,为了有成效任何饮食模式都应该是可持续的。
