JAMA Surg:吲哚青绿肺靶向吸入vs静脉注射用于肺癌肿瘤边缘的识别

吲哚青绿肺靶向吸入给药是可行的,临床上术中吲哚青绿肺靶向吸入对肺部肿瘤边缘的显示效果优于静脉注射

手术中肿瘤边缘的识别对肿瘤切除是非常重要,而静脉注射吲哚青绿(ICG)在肺癌切除术中存在一定的局限性。近日研究人员考察了一种ICG吸入术中肺肿瘤边缘显示技术的临床应用价值。
 
在小鼠和家兔肺肿瘤模型中,应用近红外成像系统研究了吸入ICG在体内外肺肿瘤边缘的分布。通过与静脉注射法(n=32,每个时间点4只小鼠)比较,评价ICG吸入肺肿瘤边缘检测的效果。基于临床前的数据,研究人员还评估了使用ICG吸入来帮助检测肺癌患者的肿瘤边缘(n=6)。
 
吸入ICG后10min至24h,ICG在肺部的分布明显高于其他器官(肺信噪比:39486.4)。体外和组织学分析表明,在两种肺肿瘤模型中,吸入ICG后,在健康肺组织中都可以观察到,但在肿瘤组织中却很少发现。正常肺组织和肿瘤肺组织荧光信号的差异是由于肿瘤引起的机械性气道阻塞和正常组织中吸入ICG后肺泡巨噬细胞摄取差异导致的。在20倍低剂量ICG下吸入,肿瘤边缘检测的效率比静脉注射高2倍(2.9)。
 
研究认为,吲哚青绿肺靶向吸入给药是可行的,临床上术中吲哚青绿肺靶向吸入对肺部肿瘤边缘的显示效果优于静脉注射。
