JCEM:NAFLD和肝纤维化与股骨骨密度降低无关

在美国年龄超过50岁的人群中,肝脂肪变性和纤维化与基于股骨DXA的骨质减少或骨质疏松症诊断无关。

非酒精性脂肪性肝病(NAFLD)是否可能是降低骨矿物质密度(BMD)的危险因素,仍存在争议,并且肝纤维化是否是NAFLD患者未来发生肝脏相关事件的主要预测因素尚未明确。

近日,内分泌和代谢性疾病领域权威杂志Journal of Clinical Endocrinology & Metabolism上发表了一篇研究文章,这项研究旨在评估美国一般人群中肝脏脂肪变性和纤维化是否与BMD降低有关。

在2017年至2018年期间的全国健康与营养检查(NHANES)基于人群的横断面分析中,研究人员同时使用了股骨颈的振动控制瞬态弹性成像(VCTE)和双能X线骨密度仪(DXA)进行检测。受控衰减参数(CAP)≥274 dB/m被认为是肝脂肪变性的指标,而中位肝劲度测量值(LSM)≥8 kPa则表明存在明显的肝纤维化。研究人员纳入了所有年龄在50岁以上,具有可靠的VCTE和股骨颈DXA检查结果的患者(925名男性和859名女性)。该研究的主要结局是股骨颈BMD值,用于评估有无骨质减少或骨质疏松症。

研究结果表明,硬脂变性和明显的纤维化在人群中非常普遍,在男性患者中分别占53.1%和9.6%,在女心患者中分别占44.2%和8.0%。在单变量分析中,无论男性还是女性,肝脂肪变性与骨质疏松症的患病率均较低,而根据肝纤维化程度没有发现差异。校正年龄、BMI、种族、烟尘和糖尿病等潜在混杂因素后,CAP和LSM均与男女性参与者BMD降低均无显著相关性。
