JCEM:长期体重变化与晚年痴呆之间的关系

近期和远期减肥都与中老年人患老年痴呆症的高风险相关,这与体质虚弱无关。

痴呆症是一种主要的神经退行性疾病,威胁着老年人的健康寿命,并给世界各地的卫生系统带来巨大的医疗负担。据估计,2018年全球痴呆症病例数量为5000万例,到2050年病例数预计将增加两倍。考虑到这种疾病不可逆的性质,因此,识别痴呆症的危险因素和早期预测因素对制定有效的预防策略至关重要。在中老年人中,体重减轻与随后患痴呆的风险较高有关。然而,大多数研究对体质虚弱的潜在影响受到了有限的随访时间或欠佳的对照限制。

近日,内分泌和代谢性疾病领域权威杂志Journal of Clinical Endocrinology & Metabolism上发表了一篇研究文章,研究人员旨在调查美国中老年人中体重变化与痴呆风险之间长期和时间关系。

该研究共有5985名50岁及以上的参与者,他们均来自健康和退休研究(HRS)。研究人员通过对1992-2008年期间进行了9次体重指数测量,并计算出了长期体重变化。然后,研究人员随访了受试者在2008年至2018年期间的痴呆状况。此外,研究人员使用多变量cox比例风险模型进行了分析。

在研究随访期间(平均为7.54年),共记录了682例(11.39%)痴呆病例。在控制了基本人口统计学和生活方式后,与体重稳定组(中位数:0.11kg/m2/年)相比,体重减轻组(中位数:-0.23kg/m2/年)患痴呆的风险显著升高(HR=1.60,95%CI为1.33,1.92)。在进一步调整体质虚弱后,这一相关性有所减弱,但仍然很强且显著(HR=1.57,95%CI为1.30,1.89)。在痴呆诊断前约14-18年的体重减轻可观察到与痴呆症之间显著的相关性(HR=1.30,95%CI为1.07,1.58),并且与更接近诊断的情况一致。

由此可见,近期和远期减肥都与中老年人患老年痴呆症的高风险相关,这与体质虚弱无关。
