J Hepatol:加拿大学者新建THRI评分系统,可预测肝硬化患者的10年肝癌风险

研究者首先应用回顾性图表分析,确定经活检或非侵入性措施诊断的肝硬化患者的推导队列。根据肝硬化的病因,对特定疾病的HCC发病情况进行评估。采用多因素Cox回归分析明确HCC相关因素,并将其用于创建预测HCC发生风险的评分系统。

结果,在≥6个月的随访后,2079例肝硬化患者中有226例(10.8%)发生HCC。HCC的10年累积发生率因病因不同而不同,从病毒性肝炎患者的22%,到脂肪性肝炎患者的16%和自身免疫性肝病患者的5%(P<0.001)。多因素Cox回归分析显示,年龄、性别、病因和血小板与HCC有关。

THRI的分数分配与每个风险比相对应,低(<120分)、中(120~240)和高(>240)风险组的10年累积HCC发生率分别为3%、10%和32%。研究者在一组PBC、HBV和HCV相关肝硬化患者(n=1144)队列中对该模型进行了外部验证,该模型在推导队列和验证队列中表现出相似的预测能力(Harrell’s c-statistic 0.77)。

研究者指出:HCC发病率因肝硬化病因不同而显着不同。使用现有临床和实验室参数的THRI在肝硬化患者中的HCC预测能力良好,且已在外部队列中得到验证。这一风险评分可能有助于完善针对肝硬化患者的HCC监测指南推荐。
