运动疗法(ET)被提倡作为慢性非特异性腰痛(CNSLBP)的治疗方法。然而,治疗效果仍然很低。在其他慢性疾病中,与低强度ET相比,高强度的训练对一般健康相关和疾病特定的结果都有较大的改善。可能,高强度的训练也能提高CNSLBP的效果大小。本研究旨在比较高强度ET项目与类似中等强度ET项目对CNSLBP患者的残疾、疼痛、功能、运动能力和腹/背肌力量的影响。

 

在一项随机对照试验中,CNSLBP患者在高强度训练(HIT)或中等强度训练(MIT)下进行了为期12周的ET计划(24节,每节1.5小时,每周两次)。在基线和训练计划结束后,进行了评估残疾(修改奥斯维斯特指数[MODI])、疼痛强度(Numeric Pain Rating Scale)和功能(Patient Specific Functioning Scale)的问卷调查,评估运动能力(V˙O2max,循环时间)的心肺运动测试,以及评估腹部/背部肌肉力量(最大肌肉扭矩)的最大等长肌力测试。

 

研究共纳入了38名参与者(HIT:n = 19,MIT:n = 19)(平均年龄,44.1岁,SD = 9.8,12名男性)。各组在基线上没有差异。MODI、V˙O2max和循环时间的组间差异(P<0.01)有利于HIT。组内在MODI、V˙O2max和循环时间上均有改善(P < 0. 01),两组在MODI(HIT:-64%,MIT:-33%)、Numeric Pain Rating Scale(HIT,-56%;MIT,-39%)、Patient-Specific Functioning Scale(HIT:+37%,MIT:+39%)、V˙O2max(HIT:+14,MIT:+4%)、骑车时间(HIT:+18%,MIT:+13%)和背部肌肉力量(HIT:+10%,MIT:+14%)上均有改善。

 

综上所述,该案就结果表明,高强度训练对CNSLBP是一种可行、耐受性好、有效的治疗方式。而且,它比中等强度下进行的类似ET在残疾和运动能力上有更大的改善。
