Alzheimers Dement:高血压和阿尔茨海默病尸检病理的系统综述

高血压和ADNC之间存在正相关,但影响可能不大,并且可能随着年龄的增加而减弱。
既往的研究表明高血压是AD和痴呆的重要危险因素,但其具体的影响机制尚不清楚。目前仅有少量的研究表明高血压可能与阿尔茨海默病神经病变(Alzheimer’s disease neuropathological changes,ADNC)有关。
研究结果
基于此,来自荷兰阿姆斯特丹神经病医学中心的Edo Richard教授团队2022年07月在《Alzheimers Dement》杂志上在线发表综述“Hypertension and Alzheimer’s disease pathology at autopsy: A systematic review”。回顾了高血压和阿尔茨海默病病理研究的相关文章,发现高血压可能与阿尔茨海默病神经病变(Alzheimer’s disease neuropathological changes,ADNC)正相关,但作用可能比较小,更多关于淀粉样蛋白、tau蛋白和血管损伤之间的相互作用可能是关键。
该研究团队回顾了Medline和Embase在尸检时评估高血压与阿尔茨海默病神经病变(ADNC)之间的关系的文献。研究的主要暴露因素是高血压,ADNC是主要的研究结果,包括:(1) 根据斑块密度测量或提交给阿尔茨海默病注册中心(CERAD)的联合会的细胞外神经炎/淀粉样斑块(NP);(2) 根据缠结密度或Braak分期测量神经原纤维缠结(NFT)。研究者在2499篇摘要中对39篇完整的EXT进行了评估,选取了15篇符合条件的文献(图1):包括来自观察性研究队列的15项尸检研究(n=5879),这些研究在人群,随访时间、高血压手术、神经病理学方法和统计学分析方面具有高度的异质性,这些研究表明,高血压似乎与较高的斑块和缠结负荷有关,但遗憾的是各个研究之间的结果并不完全一致,其中有四项研究(n=3993/5879;68%)表明高血压和ADNC之间的明确联系。另有四项研究表示,抗高血压药物可能对ADNC具有保护作用。规模较大、随访时间较长的研究报告的关系最强。

该团队研究结果表明高血压和ADNC之间存在正相关关系,但影响可能不大,而且相关性会随着高血压年龄的增加和降压药物的使用而减弱。研究斑块、缠结、脑血管病理和痴呆症之间的相互作用可能是更好地理解高血压在痴呆症发展中作用的关键。
研究者在文章中用3个大的表格详细展示了研究结果:表格1详细展示了研究设计与种群特征;表格2描述了血压和神经病理评估结果,表格3展示了详细的统计分析和研究结果。
此外,研究者还分析了细胞外神经炎/淀粉样斑块(NP)和神经原纤维缠结(NFT)相关性(图2)。以及收缩压与海马神经病理的潜在U型关系(图3)。

总而言之,该团队分析发现高血压和ADNC之间存在正相关,但影响可能不大,并且可能随着年龄的增加而减弱。本研究中,各个研究不同的统计学方法对最终的结果影响比较大。更一致的统计方法可能会使得可比性提高。
对于高血压,研究者建议可将收缩压和舒张压建模为连续变量,以优化统计能力,并且在不同的年龄亚组中,在多个时间点评估。通过评估高血压、脑血管病理损伤、NP和NFT之间的相互作用可能为高血压与痴呆的相关机制提供线索,可以更加详细地阐明上述机制如何介导高血压和痴呆之间的关系。
