JNS:脑血管痉挛的挽救性治疗

迟发性脑缺血是动脉瘤性蛛网膜下腔出血(SAH)的常见并发症,与血管造影血管痉挛和和TCD血管痉挛密切相关。对于发生aVSP或DCI的患者,建议采用挽救性治疗措施。
迟发性脑缺血(Delayed cerebral ischemia,DCI)是动脉瘤性蛛网膜下腔出血(SAH)的常见并发症,与血管造影血管痉挛(angiographic vasospasm,aVSP)和TCD血管痉挛密切相关。对于发生aVSP或DCI的患者,建议采用挽救性治疗措施,包括球囊血管成形术、动脉内输注血管扩张药物和诱导性高血压。尽管这些疗法已经使用了40多年,但几乎没有证据表明它们是安全有效的。大多数使用这些药物的证据来自单中心小样本的回顾性病例系列。此外,少数随机对照试验(RCT)也没有发现挽救性治疗对临床结局的影响。
2022年1月来自美国的Michael L. Martini等在JNS上公布了他们的研究结果,以确定挽救性治疗是否对SAH后aVSP和DCI有益。
SAH后aVSP或DCI患者的数据来自于8向临床试验和1项观察性研究。良好结局定义为3个月GOS评分为4或5。
作者共确定了1532例aVSP或DCI患者。预测性、可解释的机器学习模型显示,动脉瘤特征和神经系统并发症在预测是否采取挽救性治疗方面具有最高级别,而非入院时的神经系统评分。年轻和无脑缺血/梗死与较好的挽救性治疗结局相关,而其他重要的预后预测因素因挽救性治疗的类型(介入或非介入)而异。在基于SHAP(Shapley Additive Explanation)变量选择指导下的倾向评分匹配分析中,挽救性治疗与3个月时较高几率的GOS 4-5分(OR 1.63,95%可信区间1.22-2.17)相关。
最终作者认为,对于SAH后aVSP或DCI患者,挽救性治疗增加预后良好的几率。
