Neurology:血清基质金属蛋白酶-9与中风预后

急性缺血性脑卒中患者血清基质金属蛋白酶-9水平升高导致不良预后发生率增加

近日研究人员考察了血清基质金属蛋白酶-9(MMP-9)水平与急性缺血性脑卒中预后的关系。

研究人员对参与中国急性缺血性脑卒中降压试验的患者进行血清MMP-9水平测试(3186,男性2008,女性1178)。预后结果考察中风3个月后患者死亡、严重致残以及血管疾病发生情况。

在3个月的随访中,有767名患者(24.6%)发生严重致残或死亡。在调整了年龄,性别,发病时间,吸烟,饮酒,入院时NIHSS评分,血压,血糖,血白细胞计数,高血压药物史,高血压史,冠心病,糖尿病后,血清MMP-9与中风后死亡以及严重致残显著相关。研究发现logMMP-9每增加1个标准差(0.32 ng/mL),死亡和严重致残的综合风险提高1.16(1.06-1.28),严重致残风险提高1.12 (1.01-1.23),死亡风险提高1.29 (1.01-1.66),将血清MMP-9与传统风险因子联用可提高中风不良预后预测的准确性(净重新分类指数9.1%, p = 0.033; 综合判别的改进0.4%, p = 0.004)。

研究发现,急性缺血性脑卒中患者血清基质金属蛋白酶-9水平升高导致不良预后发生率增加。
