在一项孟德尔随机化分析中,研究者分析了20793例心血管疾病患者和27540例未患有心血管疾病受试者的数据。研究的主要结果是冠心病,所有患者进行了基因分型,用于计算加权遗传评分。研究者也分析了来自62240例试验组受试者和127299例对照组受试者的数据来评估基因与心血管疾病之间的风险。该研究的受试者中有53%为男性,平均年龄为58岁。

研究结果显示基因预测Lp(a)与冠心病风险之间的关联,与Lp(a)浓度的绝对变化呈线性关系。基因预测Lp(a)每降低10mg/dL,预示着Lp(a)与冠心病(OR = 0.942; 95% CI, 0.933-0.951)降低5.8%相关。同样,基因预测低密度脂蛋白胆固醇(LDL-C)每降低10mg/dL,预示着LDL-C与冠心病(OR = 0.855; 95% CI, 0.818-0.893)降低14.5%相关。

101.5 mg/dL的Lp(a)变化与冠心病风险之间的关联,类似于38.67mg/dL LDL-C变化与冠心病风险之间的关联。Lp(a)的降低“被证实能够将心血管疾病事件的风险降低20%”。研究者认为Lp(a)与心血管疾病之间的关系独立于LDL-C的变化(通过基因变异模拟了他汀类药物、PCSK9抑制剂和依折麦布的降低LDL-C作用)与心血管疾病之间的关系。

也有人提出质疑:“孟德尔随机化分析并不能评估LDL-C和Lp(a)降低的风险,就像有一个具有相同水平降低LDL-C或Lp(a)相同的药片获得显著的风险降低,所以孟德尔随机化分析并不能帮我们评估风险获益。”

关于该研究的一篇述评中指出有必要纳入更多Lp(a)分布差异较大的受试者群体,“在新的降低Lp(a)的试验设计时,应该更精准的聚焦在高危患者亚组中,例如那些具有较高Lp(a)水平的人群。其他的研究也应该注重Lp(a)降低对冠心病以外的其他动脉粥样硬化疾病的益处。”
