早产儿未必能从氨基酸肠外营养方案中获益

背景早产儿营养是早产儿护理的核心。目前的国际推荐倾向于高剂量氨基酸以及富含鱼油的脂质乳剂的摄入。


目的本试验的目的是比较早产儿出生24小时内基于体躯成分(1)高剂量[直接推荐每日摄入(Imm-RDI)]vs低剂量[氨基酸增量导入(Inc-AAs)]氨基酸摄入,以及(2)富含30%大豆油、30%中长链甘油三酯、25%橄榄油和15%鱼油(SMOF)的多组分脂质乳剂与根据肝细胞内脂质(IHCL)含量,基于大豆油(SO)的脂肪乳剂的作用。


设计研究者开展了一项2×2析因、双盲、多中心随机对照试验。


结果研究者对168例出生早于31周的婴儿进行随机分配。评价了133例婴儿足月时的结局。Imm-RDI与Inc-AA组之间的去脂体重无显著差异[校正平均差异:1.0 g(95% CI:−108,111 g;P=0.98)],SMOF与SO组之间的)IHCL含量也无显著差异[校正平均差异SMOF:SO比率:1.1(95% CI:0.8,1.6;P=0.58]。SMOF剂量不影响IHCL含量。2种干预在去脂体重方面有显著相互作用(P=0.05)。校正更多的混杂因素后,任一主要结局测量的组间差异之间无显著相互作用。与Inc-AA组婴儿相比,Imm-RDI组婴儿的血尿素氮浓度更可能>7 mmol/L或>10 mmol/L(75% vs 49%,P<0.01;9% vs 18%,P<0.01)。Imm-RDI组婴儿足月时的头围较小[平均差异:−0.8 cm(95% CI:−1.5,−0.1 cm;P=0.02)]。包括肥胖、肝功测试、高结合胆红素血症发病率、体重、身高、死亡率高和脑容量在内的任何预定次要结局无显著差异。


结论早产儿氨基酸Imm-RDI不能对早产儿至足月时体躯成分或生长产生获益,甚至可能有害。
