J Bone Miner Res:不同B族维生素干预 髋关节骨折风险有差异

观察性研究显示,血浆同型半胱氨酸水平的增加与骨折风险的增加相关。然而,降低同型半胱氨酸的治疗是否会影响骨折风险是未知的。2017年10月,发表在《J Bone Miner Res.》的一项研究旨在利用两项大型随机对照试验的组合数据的二次分析,调查B族维生素干预对髋关节骨折风险的影响,这两项大型RCT的原始设计为研究心血管疾病。

两项试验都具有相同的设计、干预和首要目的。基于2×2析因设计,干预由每天以下胶囊组成:叶酸(0.8mg)+维生素B12(0.4mg)+维生素B6(40mg),叶酸(0.8mg)+维生素B12(0.4mg),维生素B6单独使用(40mg),安慰剂。参与者在试验期间或延期随访期间(从试验开始到2012年末)跟踪了髋关节骨折情况。叶酸+维生素B12和髋关节骨折风险之间无统计学显着相关性,无论是试验期间(中位3.3年,HR,0.87;95% CI,0.48~1.59)还是延期随访期(中位11.1年,HR,1.08;95% CI,0.84~1.40)。试验期间,接受vs不接受维生素B6的组间,髋关节骨折风险也无显着性差异(HR,1.42;95% CI,0.78~2.61)。但是,延期随访期间,接受维生素B6的患者与不接受维生素B6的患者相比,髋关节骨折风险显着性增加了42%(HR,1.42;95% CI,1.09~1.83)。

总之,叶酸+维生素B12治疗与髋关节骨折风险不相关。高剂量维生素B6治疗与延期随访期间(试验+试验后随访)髋关节骨折风险轻微增加相关。
