J Bone Miner Res:老年男性:蛋白质摄入来源不同 骨折风险不同

研究人员研究了男性骨质疏松性骨折(MrOS)研究中的一个5875名男性(平均73.6±5.9岁)的纵向队列。在基线时,蛋白质摄入量作为总能量摄入量(TEI)的百分比被评估,所有来源的平均摄入量= 16.1%TEI。临床骨折的发病通过医生审查病历记录进行确证。

15年随访期间,总共发生612例严重骨质疏松性骨折,806例低创伤性骨折,270例髋部骨折,193例脊柱骨折,919例非髋部非脊柱骨折。研究人员使用将年龄、种族、身高、临床地点、TEI、体育运动、婚姻状况、骨质疏松、胃肠道手术、吸烟、口服糖皮质激素的使用、酒精摄入、钙和维生素D补充作为协变量的Cox成比例风险模型计算风险比(HR)及其95%可信区间(CI),所有都表示了每单位(SD=2.9%TEI)增加。较高的蛋白质摄入与严重骨质疏松性骨折风险的降低相关(HR,0.92;95% CI,0.84~1.00),低创伤性骨折也有相似的相关性。蛋白质摄入和骨折之间的相关性随着蛋白质来源的不同而具有差异;如,增加乳蛋白质和非乳动物蛋白与髋部骨折风险的降低相关(HR,0.80(95% CI,0.65~0.98);HR,0.84(95% CI,0.72~0.97)),而植物来源的蛋白质无该相关性(HR,0.99;95% CI,0.78~1.24)。蛋白质和骨折之间的相关性随着骨折部位的不同而具有差异;总蛋白质与髋部骨折风险的降低相关(HR,0.84;95% CI,0.73~0.95),但是临床脊柱骨折并非如此(HR,1.06;95% CI,0.92~1.22)。

总之,较高摄入作为TEI百分比的蛋白质(尤其是较高动物蛋白摄入)的患者具有较低的严重骨质疏松性骨折风险。
