在临床实践中和改善对血管事件的预测时,经常评估慢性肾病(CKD)措施(估计肾小球滤过率[eGFR]和蛋白尿),但是大多数主要的临床指南都没有将这些措施纳入CVD风险预测的标准化方法中。“ CKD补丁”是一种经过验证的方法,可以根据CKD度量从已建立的方程式中校准和改善预测的风险。

Matsushita等利用35个数据库的4,143,535位成年人的数据,开发了几种结合了eGFR和蛋白尿的“CKD补丁”,以通过汇总队列方程(PCE)增强对动脉粥样硬化CVD的风险预测(ASCVD),并通过系统冠状动脉风险评估(SCORE)增强对CVD死亡率的预测。CKD补丁增加的风险取决于各CKD措施与传统CVD危险因素的预期值以及eGFR和蛋白尿的危险比之间的偏差。然后,研究人员在37个独立的数据集中的4,932,824位成年人中验证了该方法,将原始PCE和SCORE方程(在每个数据集中重新校准)与添加了CKD补丁的方程进行了比较。

ASCVD和CVM风险随CKD分期的提高而增加

在极高风险(eGFR 30–44ml/min/1.73m2,蛋白尿 ≥30mg/g)、高风险eGFR 45–59ml/min/1.73m2,蛋白尿 30–299mg/g)和中风险eGFR 60–89ml/min/1.73m2,蛋白尿 30–299mg/g)的CKD患者中,使用CKD补丁预测的CVD死亡率的预测风险与使用SCORE预测的初始风险之比的中位数分别为2.64(IQI:1.89–3.40)、1.86 (1.48–2.44) 和1.37 (1.14–1.69),表明采用SCORE预测CVD死亡风险时,CKD的风险被低估了。 采用PCE的ASCVD的相应初始风险之比的中位数分别为1.55(1.37-1.81)、1.24(1.10-1.54)和1.21(0.98-1.46)。

根据可用的CKD措施,“ CKD补丁”可用于定量提高美国和欧洲主要指南中推荐的ASCVD和CVD死亡率风险预测公式。
