Lancet:P2Y12抑制剂能够挑战阿司匹林动脉粥样硬化二级预防的地位吗?

在过去的几十年中,动脉粥样硬化患者的治疗管理已经有了实质性的进展,主要是通过有效的药物治疗,如抗血栓策略。抗血小板药物的二级预防目前被推荐用于已确诊的脑血管、冠状动脉或外周动脉疾病患者。

在过去的几十年中,动脉粥样硬化患者的治疗管理已经有了实质性的进展,主要是通过有效的药物治疗,如抗血栓策略。抗血小板药物的二级预防目前被推荐用于已确诊的脑血管、冠状动脉或外周动脉疾病患者。在抗血小板药物中,阿司匹林是临床实践中被选用的药物,因为它在心血管病的二级预防中被证明具有净获益。另外,其他抗血小板药物在动脉粥样硬化患者中显示出至少与阿司匹林相似的获益。P2Y12抑制剂是最常用的抗血小板药物,可以作为阿司匹林的替代品。几项药效学研究表明,P2Y12抑制剂对血小板的抑制作用比阿司匹林更为强大。许多试验比较了P2Y12抑制剂和阿司匹林对动脉粥样硬化患者的二级预防作用,结果各不相同。

为了全面评估综合疗效和证据质量,2020年5月来自意大利的Mauro Chiarito等在 Lancet 发表了他们的系统性综述和 meta 分析结果,直接比较P2Y12抑制剂单药治疗和阿司匹林单药治疗对动脉粥样硬化患者二级预防的疗效。

在这项系统回顾和荟萃分析中,所有比较P2Y12抑制剂和阿司匹林单药治疗对脑血管、冠状动脉或外周动脉疾病患者二级预防作用的随机试验均被纳入评估。复合主要终点为心肌梗死卒中。关键二级终点为全因死亡和血管性死亡。该研究注册为 PROSPERO(CRD42018115037)。

共有9个随机试验纳入本研究,42108名患者被随机分配给P2Y12抑制剂(n=21043)或阿司匹林(n=21065)。接受P2Y12抑制剂治疗的患者与接受阿司匹林治疗的患者相比,能够降低心肌梗死风险(临界)(OR 0.81 [95% CI 0.66-0.99];I2=10.9%)。接受P2Y12抑制剂的患者和接受阿司匹林的患者相比,卒中风险(OR 0.93 [0.82-1.06];I2=34.5%)、全因死亡(OR 0.98 [0.89-1.08];I2=0%)和血管死亡(OR 0.97 [0.86-1.09];I2=0%)没有差异。同样,接受P2Y12抑制剂的患者与接受阿司匹林的患者相比发生大出血的风险(OR 0.90 [0.74–1.10];I2=3.9%)没有差异。单用P2Y12抑制剂预防1例心肌梗死的 NTT 为244例。无论使用何种类型的P2Y12抑制剂,结果都是一致的。

最终作者认为,在二级预防中,与阿司匹林单药治疗相比,P2Y12抑制剂单药治疗可降低心肌梗死的风险,而卒中的风险相似。P2Y12抑制剂单药治疗的益处是有争议的,因为预防1例心肌梗死的NNT太高,而且对全因和血管死亡率没有任何影响。
