JAHA:环境中铅、镉暴露与左心室结构和功能有关吗?

该研究结果表明,暴露于铅、镉或两者均有的环境中可能是左心室收缩功能障碍的一个危险因素,而左心室收缩功能障碍往往是心脏衰竭的重要基础疾病。

实验研究的结果表明铅和镉对心肌有着直接的毒性作用,但是由于研究设计的限制在人类上进行的,评估暴露,并使用心力衰竭作为后期阶段终点的研究相对较少。近日,心血管疾病领域权威杂志JAHA上发表了一篇研究文章,研究人员主要评估了铅和镉暴露对左心室结构和功能的影响。

在这项前瞻性人群研究中,研究人员分析了左心室功能与血液中铅含量(BPb)和24小时尿中镉含量(UCd)之间的相关性。研究人员从佛兰芒人群中随机招募179名参与者(女性占50.3%;平均年龄为39.1岁),在招募时(1985年-2000年)这些参与者BPb和UCd的几何均数分别为0.20μmol/L和6.1nmol。研究人员在11.9年(中位数)后(2005年-2010年)利用二尖瓣血流、二尖瓣环运动和斑点追踪的多普勒成像评估了参与者左心室收缩、舒张功能。

在多变量调整的线性回归中,研究人员发现左室收缩功能下降与BPb有关。对于曝光增加一倍,估计的整体纵向应变为-0.392%(P=0.034),局部纵向应变为-0.618%(P=0.028)和应变率为0.113s-1(P=0.008),和局部径向应变率为0.056 s-1(P=0.050)。局部纵向应变率(-0.066s-1,P=0.009)和局部径向应变(-2.848%,P=0.015)的下降也与UCd有关。包括暴露指标的模型并不能区别左心室功能障碍主要是与BPb还是UCd相关。左心室舒张功能与BPb或UCd相关(P≥0.159)。



虽然影响的程度很小,该研究结果表明,暴露于铅、镉或两者均有的环境中可能是左心室收缩功能障碍的一个危险因素,而左心室收缩功能障碍往往是心脏衰竭的重要基础疾病。
