Neurology: 蛛网膜下腔出血后脑自动调节功能的病理生理学差异

背景:了解蛛网膜下腔出血患者脑血管自动调节功能受损的生理基础(SAH)及其与神经预后的相关性。

方法:队列包括2010年3月至2015年5月间121例非创伤性蛛网膜下腔出血住进重症监护单元的患者。DSA确定脑血管痉挛,高分辨率CT发现新的脑梗死灶定义为迟发性脑缺血(DCI)。每天记录入院后2-4天的脑血流量和血压。通过投影寻踪回归从压力流量关系量化自动调节能力。主要的预后是与血管痉挛和DCI相关的自动调节机制的早期改变。

结果:仅43例患者有血管痉挛,9例患者发生DCI,14例既有血管痉挛又存在DCI。自动调节的能力可正确预测训练队列86%患者发生DCI,非原始模型中80%患者发生DCI。与未形成继发并发症或仅有血管痉挛的患者相比,发生DCI的患者存在远端自动调节能力。血压短暂下降,血流量变化率显著下降。血流增加率显著降低表明,尽管压力短暂增加,血流量增加的能力下降。

结论:自动调节损害的程度和性质可准确预测个体神经学并发症,并表明不同损害潜在的生理机制差异。更好地了解这些可能会制定出有针对性的干预措施,以减轻神经功能的损害。
