BMJ open:儿童和青少年高频率进食反而与低体重指数相关,建议每天吃5餐

儿童和青少年的体重指数(BMI)目前的趋势表明,在不同的低收入/中等收入国家(LMICs),儿童和青少年的体重指数持续快速增长。此外,拉丁美洲某些国家也报告了一些从体重不足迅速过渡到超重和肥胖的情况,在这些国家,学龄儿童体重超标分布中位数估计为29.8%,而在青少年中这一数字为17.8%

体重指数变化的主要原因为食物摄入和能量消耗之间的不平衡来。因此,某些饮食特征如饮食质量或者能量摄入的差异可能会影响BMI变化。从这个意义上说,进食频率被认为是体重指数增加的潜在风险因素,比如超重和肥胖

然而,有研究表明,定期(即更频繁地)进食与较低的超重和肥胖风险有关。此外,最近一项对57项观察性研究的系统回顾和元分析报告称,频繁饮食与更好的营养结果(包括较低的BMI)之间存在显著关系。但这项荟萃分析只包括来自高收入国家的报告,因为大多数总结的研究是横断面的(只有8个研究是纵向的),而且只有一个研究来自拉丁美洲(波多黎各)在这些纵向研究中,没有考虑进食频率和BMI随时间的变化。因此,进食频率与BMI之间的关系在拉丁美洲是否成立尚不清楚。

近日,BMJ open上发表的一项研究就评估了较低收入国家(秘鲁)儿童饮食频率与BMI随时间变化之间的关系。在中低收入国家,饮食频率与BMI到底是正相关还是负相关呢?

研究分为两个不同年龄段的群组:年龄较小的一组登记了20526 - 18个月的儿童,年龄较大的组包括10007 - 8岁的儿童。采用前哨点采样策略对哨点进行了多阶段、聚类分层、随机抽样技术。最初的样本框架是在地区一级进行的,从1818个可供选择的地区中选择了20个哨点。为了对贫困地区进行过采样,抽样过程中没有包括最富裕的5%地区。

在研究中,父母需回答与儿童进食相关的7个问题,问题如下:在之前的24小时内,儿童是否在早餐前进食:任何食物?有早餐吗?早上和中午有什么食物吗?有正餐吗?中午和晚上有吃的吗?有晚餐吗?晚饭后有吃的吗?对这七个问题简单地加上/的回答,用来估计孩子在过去24小时内进食的总次数。为便于分析,变量被分类为每天<44567次,5次,中间类别(每天5次)作为参考类别。

其他变量也被纳入回归模型中作为基线评估的潜在混杂因素,包括:性别(男孩或女孩)、年龄(以年为单位)、基于家庭资产的财富指数评估分为三等级(低、中、高),环境类型(城市或农村),母婴教育年限(<7年、711年和12年)等。

研究结果:研究最初共招募了2052名较年轻的儿童。其中103(5.0%)失访,1(0.1%)因数据缺失而被排除。因此最终有1948人被纳入基线分析,平均年龄为4.3岁岁, 49.6%为女性。基线数据显示,这些儿童平均每天进食4.9(标准差:1.0,范围:2 7) 43.5%每天进食5次,11.0%每天进食4次, 4.0%每天进食7次。值得注意的是,儿童的年龄、母亲和父亲的教育、社会经济地位、环境类型和母亲的BMI与饮食频率呈正相关。

根据基线饮食频率分类的研究人群特征。

根据多变量模型,较低的进食频率与较高的BMIBMI-for-age z分数有关。每天吃4顿饭的儿童体重指数(BMI)增加得更多。然而,那些每天吃67次的人的变化没有显示出相关性。

总之,儿童每日进食频率与体重指数随时间的变化之间存在关联。尽管这种关联的规模很小,但随着时间的推移,低频率进食与更大的BMI增加有关:每天进食少于5次的人有更大的BMIBMI-for-age z-score测量值,而每天进食5次的人在这些结果上没有增加。此外,几乎三分之二的受试者在基线检查时(即45岁的儿童)每天进食5次,但当儿童在年龄增长到1415岁之间时,这一数字减少到不到一半。

食物频率对内分泌反应和调节的影响可以解释该发现。增加进食频率可能会减弱餐后代谢和内分泌对营养摄入的反应,进而导致胰岛素分泌减少由于有更多的证据表明,不吃饭,特别是早餐,会增加儿童营养过剩(即超重和肥胖)的风险,因此应提倡每天至少五餐的规律饮食模式。此外,还必须向儿童,特别是他们的父母、家人和照顾者宣传膳食的质量和组成。
