据《自然》发表的研究Phylogenetic ctDNA analysis depicts early stage lung cancer evolution显示,通过分析血液中的特定遗传标记,可以对肺癌的演变进行监测。研究表明,在血液样本中发现的循环肿瘤DNA (ctDNA)与组织亚型和其它变量(包括病情复发)相关。这些发现表明,液体活检或可提供一种非侵入式方法来分析肺癌患者,这也许有助于改进治疗方法,限制肿瘤复发。

肺癌难以治愈,是全球范围内主要的癌症致死原因。已成型的转移性非小细胞肺癌(NSCLC)单靠化疗无法治愈,而在手术摘除肿瘤组织后进行化疗也只能提高约5%的患者的生存。改进术后治疗是必要的,而找出方法来判断患者是否可能复发或许有助于优化治疗决策。

英国弗朗西斯·克里克研究所的Charles Swanton及同事在TRACERx肺癌临床试验研究中,前瞻性地监测了100位转移性非小细胞肺癌患者从确诊到复发和死亡的演变过程,他们使用定制的测序面板分析了在术前和术后进行的多次液体活检。他们发现检测ctDNA可以预测治疗结果,如对术后化疗的抗性,在某些情况下还可以比传统临床方法更早地发现复发情况。因此,分析液体活检可能有助于增强识别有可能从术后化疗中受益的患者,并能跟踪复发期间不同癌症克隆和亚克隆的演变。

该方法的局限包括ctDNA分析的成本(分析单个患者5个样本的费用为1750美元)和NSCLC筛选的敏感度,后者可能受到肿瘤大小的限制。但是经过适当验证后,结果表明通过液体活检检测ctDNA有望简化对癌症发展和演变的监测。
