Eur Respir J:床旁肺超声对COVID-19患者危险分层的评估和治疗指导

肺超声和LUZ评分是一种简单、方便、快速的床旁超声工具,用于识别入院时因COVID-19导致的严重肺损伤.

早期发现新冠肺炎患者的肺部受累和呼吸系统并发症对临床医生有巨大帮助,以便对患者进行个性化管理并预测机械通气的需要。然而,COVID-19存在诊断挑战,因为许多患者表现出症状和放射学检查结果之间的不一致。肺超声可用于评估COVID-19引起的肺损伤。然而,在住院患者中,肺超声评估肺损伤的预后意义和时间线变化尚不清楚。

发表在Eur Respir J杂志的一篇文章采用前瞻性队列研究,使用定量量表(肺部超声(LUZ)评分)分析入院72小时内肺超声对新冠肺炎患者的预后价值。主要终点是院内死亡或进入重症监护病房治疗。次要终点是住院总时间,入院72小时内氧气流量的增加和医疗费用的增加。

最终纳入130例患者,年龄为56.7±13. 5岁。从症状开始到入院的中位数时间为6(4-9)天。通过LUZ评分评估的肺损伤在最初72小时内无差异(入院时为21(16-26)分,72小时为20(16-27)分;p=0.183)。单变量logistic回归分析,PaO2/FiO2(PAFI)(HR=0.99,95%CI, 0.98-0.99; p=0.027)和LUZ评分>22分(5.45,1.42-20.90;p=0.013)是主要终点的预测因子

肺超声和LUZ评分是一种简单、方便、快速的床旁超声工具,用于识别入院时因COVID-19导致的严重肺损伤。入院前72 h评分无变化强调入院后应进行首次超声评估的重要性。入院基线LUZ评分 > 22是入住ICU或住院死亡的预测因素。该评分有助于早期实施或强化COVID-19感染的治疗。LUZ评分可与临床变量(由PAFI估计)相结合以进一步细化患者的风险分层。
