ARD:髋关节或膝关节置换术前行神经肌肉锻炼有良好术后效应

本试验是研究者单盲的随机对照试验,共纳入165例因严重关节炎拟行髋关节或膝关节置换术的患者。165例患者随机分组,干预组在进行标准的关节成形术前,接受术前监督性神经肌肉锻炼辅助治疗,辅助治疗2周一次,每次1小时,为期8周;对照组仅进行标准的关节成形术治疗。主要观察指标是患者身体功能的自评情况,对髋关节炎和膝关节炎的患者分别使用髋关节伤残和骨关节炎评分问卷、膝关节损伤和骨关节炎评分问卷中的日常生活活动能力分量表进行评估。试验的终止时间是术后3个月。

本试验研究对象平均年龄67±8岁,其中84例(51%)髋关节炎患者,92例(56%)女性患者。153例(93%)患者按计划行手术治疗并接受术后评估。髋关节炎患者和膝关节炎患者术后疗效无统计学差异(p=0.7370)。干预组和对照组在术后三个月时日常生活活动能力(OR值4.4,95%可信区间0.8-9.5)及疼痛程度(OR值4.5,95%可信区间0.8-9.9)无差别。然而,统计学结果显示,术前接受神经肌肉锻炼辅助治疗干预组在整个试验观察期内(从基准时间到术后3个月)的术后疗效较对照组有显著差异(p=0.0029)。

术后三个月时,与单纯的髋关节或膝关节全关节置换术相比,进行术前为期8周的监督性神经肌肉锻炼无额外疗效;然而,干预组在短期日常生活活动能力和疼痛缓解改善方面在统计学上有显著意义,且干预组术后日常生活能力的恢复与疼痛改善更早。
