背景:尽管在预防和控制IHD上取得了巨大进步,但IHD仍然是全球发病和死亡的主要原因。饮食在IHD中起到关键作用,但是关于IHD中饮食因素的作用的综合性描述至今还未完全完成。

目的:本研究旨在检验长期假设——对IHD铜具有保护作用,锌具有有害作用。

设计:研究者使用了具有遗传工具(孟德尔随机)的分离样本工具变量分析。研究者从全基因组关联研究中获取了单核苷酸多态性(SNPs),与红细胞铜和锌密切(P值<5×10-8)且独立相关。研究者将这些铜和锌的遗传预测因素应用于最大、最广泛的基因型IHD病例(n≤76014)-对照(n≤264785)研究,并主要基于CARDIoGRAMplusC4D 1000 Genomes和UK Biobank SOFT CAD,从而获得铜和锌对IHD影响的SNP特异性Wald评估值,同时通过使用逆方差加权进行组合。敏感性分析使用了MR-Egger方法,二次分析在P值<5×10-6时考虑了与红细胞铜和锌独立相关的SNPs因素。

结果:遗传工具分析表明铜与IHD负相关(OR:0.94;95% CI:0.90,0.98)。遗传工具分析表示锌与IHD正相关(OR:1.06;95% CI:1.02,1.11)。通过MR-Egger的敏感性分析未发现未知的多效性;弱相关的SNPs给出了与铜类似的结果。

结论:遗传工具析验证了长期的假设,进一步研究对IHD的影响,尤其是铜,可能发现降低死亡率和发病率主要原因的办法。
