BMJ:英国一线降压药物的降压效果评估

英国初级保健数据显示,对于无糖尿病的高血压患者,使用钙通道阻滞剂的降压效果优于血管紧张素转换酶抑制剂/血管紧张素受体阻滞剂

近日研究考察了,根据英国高血压临床指南,基于年龄和种族的治疗建议对患者当前常规临床护理中血压的影响。
 
本次研究为观察性队列研究,新近开始接受血管紧张素转换酶抑制剂/血管紧张素受体阻滞剂(ACEI/ARB)、钙通道阻滞剂(CCB)和噻嗪类降压药的患者参与,根据年龄(55岁以下或以上)和种族(黑人或非黑人)分层,在基线检查、12周、26周和52周进行随访,考察ACEI/ARB、CCB与噻嗪类新使用者的血压变化。
 
总计87440名ACEI/ARB新使用者、67274名CCB新使用者和22040名噻嗪类新使用者参与研究,参与者每日血压测量中位次数为4。对于没有糖尿病且年龄小于55岁的非黑人患者,12周时,使用CCB与ACEI/ARB的患者相比,收缩压额外下降1.69 mm Hg,55岁及以上人群收缩压额外下降0.40 mm Hg,对75岁及以上的人群,使用CCB的收缩压下降幅度更大。在黑人中,使用CCB后血压的下降幅度显著高于ACEI/ARB(差异为2.15 mm Hg)。
 
英国初级保健数据显示,对于无糖尿病的高血压患者,使用钙通道阻滞剂的降压效果优于血管紧张素转换酶抑制剂/血管紧张素受体阻滞剂。
