Clin Chem:平衡血管紧张素II在原发性醛固酮增多症诊断中的应用

在筛选原发性醛固酮增多症(PA)时,许多药物(包括大多数抗高血压药物)和生理因素会影响醛固酮/肾素比率(ARR)。

在筛选原发性醛固酮增多症(PA)时,许多药物(包括大多数抗高血压药物)和生理因素会影响醛固酮/肾素比率(ARR)。本研究试图验证一种新的平衡血管紧张素II (eqAngII)检测方法,并比较在肾素-血管紧张素系统受影响的条件下,醛固酮/血管紧张素II比值(AA2R)与当前ARR之间的相关性。

研究纳入的78例患者,其中有22例经PA排除,56例经氟氢化可的松抑制试验(FST)证实为患者。检测外周血eqAngII、血浆肾素活性(PRA)和直接肾素浓度(DRC)。

研究结果显示,EqAngII与DRC和PRA的一致性良好,与PA的诊断、姿势和氟氯可的松的使用无关。EqAngII在FST时与DRC (r = 0.691)和PRA (r = 0.754)有密切(P < 0.01)的相关性。在312个样本中,DRC和PRA的功能灵敏度分别低于43.9%和15.1%,而eqAngII的灵敏度仅为7.4% (P < 0.01)。Bland-Altman分析显示,在低肾素水平的样本子集中,与eqAngII相比,PRA和DRC被高估了。无论是通过DRC(r = 0.878)还是PRA(r = 0.880)测量肾素,AA2R不仅显示出与ARR一致的变化,而且与ARR密切相关(P <0.01)。

研究结果表明,eqAngII和AA2R的动态变化与肾素和ARR具有良好的一致性和密切的相关性。eqAngII法比DRC和PRA法有更好的敏感性,特别是在低浓度下。AA2R是否能降低一些影响ARR诊断能力的因素的影响,值得进一步研究。
